📈 Is there an optimal blood ketone level for weight loss on a ketogenic diet?
This study followed 217 adults with overweight or obesity who measured their blood ketones and body weight every day during a 14-day ketogenic diet. The goal was to identify the blood ketone level most strongly associated with successful weight loss.
Researchers found:
✅ A blood β-hydroxybutyrate (BHB) level of ≥0.5 mmol/L was the strongest predictor of weight loss.
✅ Participants reached this level in an average of 7.7 days, with nearly 60% achieving it between days 5 and 8.
✅ Average weight loss was 7.0% in just 14 days.
✅ Every additional day spent with BHB ≥0.5 mmol/L was associated with about 0.5% greater weight loss.
✅ The Mediterranean ketogenic diet with phytoextracts helped participants reach ketosis faster than the classical ketogenic diet.
The findings suggest that monitoring blood ketones may help optimize weight loss by ensuring nutritional ketosis is achieved and maintained.

A recent meta-analysis suggests that ketogenic diets may improve both hormonal and metabolic outcomes in women with polyendocrine metabolic ovarian syndrome (PMOS; formerly PCOS) and overweight or obesity.
Researchers analyzed data from 11 studies (4 randomized controlled trials and 7 pre-post intervention studies) evaluating ketogenic dietary interventions, including very-low-calorie ketogenic diets and other carbohydrate-restricted approaches.
Key findings:
🔹 Total testosterone decreased significantly
🔹 Sex hormone-binding globulin (SHBG) increased significantly
🔹 LH and the LH/FSH ratio improved
🔹 BMI, waist circumference, body fat, fasting insulin, and insulin resistance decreased
🔹 Triglycerides and total cholesterol decreased, while HDL cholesterol increased
🔹 Several studies also reported improvements in menstrual regularity, ovulation, and fertility-related outcomes
Interestingly, reductions in testosterone were not explained by changes in body weight or insulin levels, raising the possibility that ketosis may influence ovarian function through additional mechanisms.
While the findings are very promising, larger randomized controlled trials are needed to confirm these effects.

Continuous glucose and ketone monitoring was recently approved by the FDA for people ages 2+ living with diabetes, an important milestone for metabolic health.
The ability to see glucose and ketone dynamics continuously opens up new possibilities, particularly for people who need high-frequency information to make important health decisions.
This reinforces something we’ve believed at Keto-Mojo for a long time: glucose alone doesn’t always tell the whole story.
Looking at glucose alongside ketones and understanding your Glucose Ketone Index (GKI) can provide additional context about your metabolic state.
Continuous and point-in-time monitoring can serve different needs. Continuous monitoring can be valuable when high-frequency tracking matters. Fingerstick blood testing provides an intentional, on-demand checkpoint when you want to understand where you are at a specific moment.
We applaud the continued innovation happening in metabolic monitoring and the greater visibility it can give people into their own metabolic health.

Can a low-carb ketogenic diet be safely used in patients with heart failure and obesity?
This retrospective study followed 125 adults with heart failure, including both reduced (HFrEF) and preserved (HFpEF) ejection fraction, who adopted a ketogenic diet for at least one year in a clinical weight-loss setting.
Key results:
▪️ Median weight loss: 24.7 lb (11.2 kg) over approximately two years
▪️ HF hospitalizations dropped by about 50% (rate ratio = 0.5), especially in HFpEF
▪️ HDL cholesterol increased, triglycerides decreased; LDL and total cholesterol remained stable
▪️ 45% of patients with type 2 diabetes reduced their insulin dose
▪️ No significant changes in ejection fraction, diastolic function, or mitral regurgitation
▪️ ACE inhibitor, ARB, or ARNI doses were reduced in 39% of HFrEF patients
These findings suggest a ketogenic diet may be a safe, effective strategy for weight loss and metabolic improvement in patients with heart failure and obesity, while also lowering hospitalization risk.
While randomized trials are needed to confirm long-term safety and benefits, these findings are encouraging.

Can a ketogenic diet influence tumor biology in endometrial cancer?
In a randomized feasibility trial, women with overweight or obesity and newly diagnosed endometrial cancer followed either a very low-carbohydrate ketogenic diet (VLCD) or their usual diet for approximately 4 weeks before surgery.
The ketogenic diet proved feasible and well tolerated:
🔹 91% of provided ketogenic meals were consumed
🔹 Nutritional ketosis was achieved within days, with mean β-hydroxybutyrate (BHB) levels reaching ~1.9 mmol/L
🔹 Only mild adverse events were reported; no serious adverse events occurred
Participants in the VLCD group also experienced notable metabolic changes:
🔹 5.5% weight loss in ~4 weeks
🔹 22% lower fasting glucose
🔹 60% lower fasting insulin
🔹 Reductions in HbA1c, C-peptide, IGF-1, and leptin
Beyond systemic metabolism, researchers observed reduced tumor PI3K signaling and increased CD8⁺ T-cell infiltration at the tumor margin.
This was a small feasibility study and was not designed to assess cancer outcomes. However, the findings suggest that short-term carbohydrate restriction may influence both metabolic and tumor-related pathways in endometrial cancer.

When weight loss is matched, does diet composition still matter?
In this randomized clinical trial, 55 adults with obesity, prediabetes, and fatty liver were assigned to ketogenic, Mediterranean, or very-low-fat plant-forward diets. Metabolic outcomes were compared after ~10% weight loss.
🔹 Liver fat fell 67% with the ketogenic diet vs 45% with both other diets.
🔹 Hepatic insulin sensitivity improved 2–3 times more with the ketogenic diet.
🔹 24-hour insulin levels fell 74%, compared with 44% and 27% on the Mediterranean and very-low-fat diets.
🔹 Prediabetes remission occurred in 50% of the ketogenic group vs 29% and 7% in the other groups.
The findings suggest that macronutrient composition may influence the metabolic benefits of weight loss, particularly liver and glucose metabolism.

Can a nutrition-focused, remote care program reduce liver disease risk in high-risk adults?
In a large U.S. matched cohort study (n=10,062), Virta Individualized Nutrition Therapy (VINT) – a telemedicine program designed to achieve nutritional ketosis and promote metabolic health – was compared with usual care.
Over an average of 2 years, the VINT participants had a consistently lower incidence of liver-related outcomes:
🔹 ~39% lower relative risk of any liver-related diagnosis (HR 0.61)
🔹 ~62% lower relative risk of progressive liver disease (MASH and beyond; HR 0.38)
🔹 ~67% lower relative risk of advanced liver disease (HR 0.33)
🔹 ~75% lower relative risk of liver-related complications (HR 0.25) 🔹Greater weight loss was also associated with lower risk: ≥15% weight loss, ~34% lower relative risk (HR 0.66)
Despite similar medication use and healthcare utilization, fewer liver-related events were observed in the VINT participants.
These findings support a lifestyle-first, metabolically targeted approach as a scalable strategy to reduce liver disease risk.
