STEMI… or Not? The Post-ROSC ECG Mystery 🚨
When a patient achieves ROSC after cardiac arrest, the first out-of-hospital 12-lead may scream STEMI… but what if two-thirds of those “STEMIs” are gone by the time they hit the ED? 🤯

What an interesting thread. While I agree position A is correct I would argue against the statement that correct AL pad placement can double survival. This has never been shown under the scrutiny of a randomized controlled trial and to be more controversial we will soon publish data from the DOSEVF trial that the AP pad position may be optimal for defibrillation which supports earlier work by Lupton et al. jamanetwork.com/journals/jam…. Stay tuned!
Defib pad placement is important. Positioning them correctly can double someone’s chance of survival.
Side A is correct. The key difference is that the pad on the left hand side of the chest has been placed on the side of the rib cage, closer to the armpit.

ALT The image shows a side-by-side comparison of two defibrillator pad placements on a CPR training mannequin, labeled "A" and "B." The background is dark blue, and the text reads: "Spot the difference. Do you know the best defib pad placement?" In image A, one pad is placed on the mannequin's upper right chest, while the other is positioned on left hand side of the rib cage, close to the armpit. In image B, one pad is also placed on the upper right chest, but the other one is placed on the left hand side, further away from the armpit and almost on top of the ribcage.

Starting January 1, 2025, Florida’s new Halo Law establishes a 25-foot exclusion zone around first responders actively performing their duties, regardless of location. Anyone who ignores verbal requests to back away, and continues to intentionally impede, threaten or harass first responders can face criminal charges. The goal is to ensure that first responders can perform their essential roles without unnecessary interference or danger.

You’re gonna only hear this on X because Mainstream Media in the Department of Public Health and City Hall has been lying to the public. You have a EMT a First Responder who feet is on the ground in this community telling you he goes on 50 to 60 overdose calls a day he say the city is full of bullshit. I’mma let you hear his conversation and let you judge for yourself. Who would you believe somebody that’s here or somebody pushing a pen and paper

The @dcfireems team under the leadership of Holly O’Byrne and @EMSPhysician David Vitberg has saved many lives, again disproving the myth that urban agencies with short transport times shouldn’t carry blood. This will be an incredible webinar!
Does your EMS system carry #WholeBlood?
In only 7 months @dcfireems has administered blood to 162 patients BEFORE they arrived at the hospital.
Of the patients who weren't already in cardiac arrest, 93% survived.

This paper received lots of media when it was published this year, and for good reason.
Join @TannerSmida and I this Friday and hear from Dr. David Dillon, first author on the @JAMANetworkOpen paper.
A reminder on key survival stats from the paper:
➡ 34.5% of patients who received naloxone achieved ROSC
→ Compared to 22.9% in the non-naloxone group.
➡ 15.9% of naloxone-treated patients survived to hospital discharge
→ Compared to just 9.7% in the non-treated group.
