Thanks @vaibhavbetter for the kind words and one of the apt description of what we are building at @FOXOclub
I just upgraded my annual blood test with FOXO Primer and got more than I imagined. Because of an autoimmune issue, I’ve had to do more “understanding” of my blood tests than most people, who often look at ranges, underlines, and move on. My annual tests were already more than standard packages, and I had learned to understand inflammation trackers and other markers earlier than I would have liked :) But I knew there was more to know. The challenge was that going beyond standard or expanded tests was either too expensive, too fragmented, or missing the most important layer: someone looking at everything holistically and giving advice that worked across systems. Over the last few years, this has changed globally. Access to extensive biomarker testing has become easier, cheaper, and increasingly paired with medical professionals who know how to correlate results. FOXO brought this to India with FOXO Primer, a comprehensive 200+ biomarker assessment that combines clinical history, system-level analysis, and practical guidance to help you understand how your body is functioning and what you can do to improve it. I was genuinely impressed by the overall experience. We invested in FOXO at the founding stage because we had been clear on this opportunity for a while. But hearing founder Subhendu’s personal journey of “fixing his health”, the journey that led to FOXO, made the mission feel even more real. Nothing drives mission orientation like a personal story, and I could relate to his too. I believe the future of preventive healthcare will be driven by companies like FOXO: helping us understand our health earlier, act sooner, and hopefully avoid many reactive doctor visits later. Think of them as our future “pre-GPs.” Worth exploring if you’ve been curious but sitting on the sidelines: foxo.club/primer

There has been some chatter on AQI and longevity thanks to the recent episode of WTF podcast. It is good to see these topics being discussed and the awareness created for a broad audience.  Thanks to @nikhilkamathcio and team. As a builder in the longevity space, I had to write about a few points that I felt were incorrect. This post is a summary of what I felt was well covered, and what was incorrect. The good parts 1. One shouldn’t supplement without any testing. AGREE. But one should also question why a certain vitamin or mineral is low and work on the root cause of that deficiency. If you have low B12 deficiency and are a regular meat eater, the root cause of deficiency could be an infection, absorption issues, or a genetic disorder of TCN2. 2. Current healthcare is sick-care in disguise and the new age medicine will be precise, preventive, personalized, and data-driven. However one question always arises how much data is enough. I think diagnostics that help us to monitor the 4 horsemen as mentioned by Peter Attia is a good starting point. 3. Wearables like smartwatch, rings, trackers have become a fashion statement and people who wear them are unaware of most of the metrics and don’t know how to improve/change them. This is where a big opportunity is. 4. Health data in India is scattered and the future founders need these data sets to build healthcare AI. We do see this challenge at @FOXOclub and are working towards it. What I disagree with I think CGM is a great tool not only for a diabetic, but for anyone who wants to manage their health. Blood glucose spikes beyond your baseline because of a certain meal, makes your insulin overwork and leads to inflammation. This is one of the hallmarks of ageing. Hence it creates an opportunity for every person to optimize their meals and health. Indiscrepancies 1. Alcohol doesn't have formaldehyde. It can happen due to unsound manufacturing processes. Ethanol (drinking alcohol) is broken down into acetaldehyde by the enzyme alcohol dehydrogenase (ADH) in the liver. 2. DEXA scans don’t measure magnesium levels directly. To measure magnesium levels, recommended methods are - Serum Magnesium, RBC Magnesium, Urinary Magnesium, Magnesium Load, Hair or Tissue Magnesium Analysis. 3. Essential Amino Acids are not produced by the body but have to be consumed from food. That’s the reason they are called essential. 4. The text overlay which mentioned prebiotics as the living organisms is incorrect. Prebiotics are fibre which we get from food, and probiotics are actually the living microorganisms. Over the conversations I've had with many people over the last few months, I realised that many don't have a good idea of what longevity is. @prashanthp mentioned correctly that it is beyond just eating, exercising, and sleeping right. In a subsequent post, I will give my perspective of what longevity is, or rather it should be. CC @gehani @sanmaya @arpiit @jatinmadhra

An annual heart checkup protocol even your doctor might not suggest! The recent news pushed me to think about how I landed up here – taking regular tests (often not recommended by doctors, but important) and making decisions with a long term perspective – not 5, but 50 years. It was not planned; I had a big health scare in 2020 that forced me to take a look at what was happening to my body. A lot of people are on the same boat – just one scare away from getting a reality check. I learned it the hard way. But we all don’t need to. Cardiac issues, particularly, are on the rise. They are no longer just about bad habits or unhealthy lifestyles – they’re often hardwired into our genes. The Diet-Heart Hypothesis from Dr. Ancel Keys in the 1950s, which demonized saturated fat, has long been debunked. Yet, we’re still stuck in that debate. It’s time we move beyond simplistic theories and embrace systems biology, where every part of our body is interconnected, and every solution needs to be personalized. What is the solution? On a fundamental level, it is to adopt systems biology. But as an immediate next step, it is to have a comprehensive annual testing protocol that digs deeper than the “full body check up” your doctor or insurer might recommend. Here’s a system that looks at your genetics, your blood markers, and your risks – not just for now, but for the future. Step 1: Genetic testing to decode your blueprint Genetic testing is the first step in understanding your cardiovascular risk. This isn’t just about single mutations but also polygenic risk scores – how multiple genetic variations combine to influence your health. Key genetic areas to test include: Lipid Metabolism: LPA, APOE, APOB Inflammation and Vascular Health: CRP, IL6 Clotting/Thrombosis: Factor V Leiden, Prothrombin, MTHFR Hypertension: ACE, AGT Medication Metabolism: SLCO1B1, CYP2C9 & VKORC1 Emerging Markers: PCSK9, SORT1 (We’ll save the deep dive into these mutations for another day.) Step 2: Blood biomarkers – Your annual dashboard Once you’ve mapped your genes, regular monitoring of key biomarkers helps track how your body is functioning in real-time. These should be measured annually or biannually for those with higher risks. Basic Lipid Profile: LDL (Low-Density Lipoprotein): “Bad cholesterol,” linked to arterial plaques. HDL (High-Density Lipoprotein): “Good cholesterol” that clears LDL. Triglycerides: Fats in the blood; excess increases heart disease risk. VLDL (Very Low-Density Lipoprotein): Precursor to LDL, carries triglycerides. Advanced Lipid Markers: Apolipoprotein B (ApoB): Measures atherogenic particles. Apolipoprotein A1 (ApoA1): Reflects HDL efficiency. Lp(a) (Lipoprotein(a)): A genetic driver of atherosclerosis. Cardiac Stress and Vascular Health: Troponin I: Detects silent heart damage. hs-CRP: Measures chronic inflammation, a key risk factor. Homocysteine: High levels impair vascular function. Omega-3 Index: Tracks anti-inflammatory fatty acids. Metabolic Health: Fasting Insulin: Detects early insulin resistance. HOMA-IR: Estimates insulin sensitivity. For individuals over 40 – or younger with significant risk factors – advanced imaging can provide critical insights: Calcium Score: Detects calcified plaques in coronary arteries. If you have a check-up planned, ask for these to be included. P.S. We at @FOXOclub are building a comprehensive testing protocol which looks beyond the traditional methods and helps you live LIFE++ CC: @gehani @sanmaya @iam_supratik999 @sarojsahoo03 @ChandniDD

Hi Everyone, @bryan_johnson is in India. And longevity is the new buzzword. Only one of these statements is true. In 2020, I realised my body was older than I could’ve comprehended. And that's where my story begins. In July 2020, I got COVID. A full body check-up that year revealed a few things – High cholesterol High triglycerides Low B12 (126 pg/ml) Low D3 And pre-diabetic. I took the first steps – reduced my alcohol consumption and focused on nutritious meals. Despite the LOW FODMAP diet, my stomach issues escalated. Another round of tests (endoscopy and ultrasound) told me that I was a walking store of diseases. A bacterial infection H. Pylori. Ulcer. Fatty liver. GERD. Villous atrophy. The standard protocol for H Pylori infection is to take 14 days of antibiotics, four antibiotics every day. I did that. And I felt better for some time. Until I lost 11 KGs in 1 month is not. While having 3 large meals and 2 snacks. No calorie deficit of any kind. I did another round of tests to rule out Tuberculosis and the deadly C word. The tests came out negative and my gut was in very good shape. The doctor said it was in my head. Anti-depressants were an option (took them for 3 days) but I still felt miserable. I needed answers. This was when I came across systems biology (I was clear that it was the future of health care). Did a metabolomics test and - Mitochondrial dysfunction Lactate of pyruvate ratio was 338 (it should be 10-20) IBS diarrhoea. Low platelet levels and a rate of decline of 10K X 10^3/ uL per month The traditional sickcare model was failing me. So I went back to studying. And got in touch with a functional medicine expert in India. For the first time in 18 months, I was equipped with more data and a personalised protocol that looked VERY different from 4 nutritionists and 5 doctors I worked with. Cut to 2023, my biomarkers were in the optimal range (and continue to be today). I stopped taking most of my supplements. My stomach was healed. My absorption started to get better. My B12 – 432 pg/ml. The “personal” spilled into work. And this time, it wasn’t a sabbatical but the second stint of life. I met my co-founders @gehani and @sanmaya , and we set out to find a deeper thesis on what it will take to build a world-class longevity startup out of India. In the last few months, we’ve run pilots with real users by getting them to do blood tests on 150+ biomarkers and working closely with them to understand their issues. Majority of the users? Diabetes. Gut issues. Auto-immune conditions. Cardio-vascular issues. It told us one thing – 30s are the new 50s. Today, we’re designing a system for a long life. Only for the people who know they want to live a long life and are here to find out how. Thanks to @bryan_johnson, the world is ready for this! P.S. He is in India this week and speaking in Mumbai today with @deepigoyal. Not a better day than this to launch FOXO and make Indians live longer and unlock Life++.

Observing how doctors talk with their patients is magic. There's a lot of psychology in the diagnosis and care involved than just science. Good doctors have to let patients not worry much about the disease and yet take care procedures seriously enough.
