Thank you Dr. Ray for sharing your story. DEI must not become a cliche. It takes real effort to effect change in a bigoted system. rrayjr.blog/2023/06/15/racis…

This is a key point I forgot to make: Make a dx. HTN Emergency is not a dx. It is a wastebasket of diagnoses. SAH, dissection, eclampsia, etc. are diagnoses. They have clear goals of care and recommended meds. NONE of which is “10-20% lower than what triage got.”

Today’s news that Envision Healthcare initiated a Chapter 11 bankruptcy filing underscores the risks of CMG influence in EDs. Envision’s bankruptcy filing does not change the status of our lawsuit. Here's why: bit.ly/3W3LoRZ #EmergencyMedicine #MedTwitter

Unclear to me why a GRACE guideline that is suppose to help define “sensible and prudent” care is making what are self described as “aspirational” recommendations.
Finally @GRADE_WG based emergency medicine/patient-centered clinical practice guidelines for diagnosis & management of acute dizziness - see #GRACE3 onlinelibrary.wiley.com/doi/… & supporting systematic reviews in May 2023 @AcademicEmerMed onlinelibrary.wiley.com/toc/… @CORD_EM @SAEM_RAMS

There's also particular medicolegal danger for regular docs in proposing these as "reasonable and appropriate", with the implication those without expert HINTS training or rapid access to MRI are inadequate. We ought not promote "zero miss" as the only appropriate care.

"EM physicians writing for EM physicians" doesn't automatically get a free pass if they've strayed into an academic fantasyland of supposed "perfect" care; a new paradigm and approach ought be tested and proven demonstrably better than our present care to be proposed.

Not a great study — @smithECGBlog and @PendellM wrote a commentary on this paper pointing out the limitations and I believe not comparing to QRS amplitudes pubmed.ncbi.nlm.nih.gov/3687…
