🧵 Presented today at #LIVES2026:
In mechanically ventilated adults with #AcuteHypoxemicRespiratoryFailure, an inhaled sedation strategy using isoflurane with intravenous adjuncts as needed did not reduce hospital mortality vs intravenous sedation. Ventilator-free days, intensive care unit–free days, hospital-free days, and 3-month quality-of-life and disability scores were also similar between groups.

Sodium bicarbonate did not improve sustained return of spontaneous circulation, survival, or neurologic outcomes in adults with in-hospital cardiac arrest compared to placebo.

is there an association between operator−patient sex, and outcomes in patients undergoing PCI? our work led by @ayayo_sharon and @TaxiarchiVicky using national @BCIS_uk data suggests that there may be.
➡️We included 138,112 consecutive PCI procedures in patients presenting with ACS in England and Wales, 2017−2020.
➡️ Male operators undertook the majority of PCI procedures; 130,604 (94.6%).
➡️Risk‐adjusted data found that, when treated by male operators, female patients experienced higher MACCE andmortality rates than male patients (4.1% vs. 3.7%; RR: 1.11 [95% CI: 1.05,1.18] and 3.1% vs. 2.7%; RR: 1.15 [95% CI: 1.08, 1.22]).
➡️Patient sex‐based differences in outcomes were not present for female operators
➡️ Operator−patient sex discordance was associated with lower transradial PCI (RR: 0.96 [95% CI: 0.95, 0.96]), lower prasugrel use (RR: 0.88 [95% CI: 0.82, 0.94]), higher MACCE (RR: 1.12 [95% CI: 1.06, 1.18]) and mortality (RR: 1.14 [95% CI: 1.07,1.21]) compared with operator−patient sex concordance.

Huge congratulations to Professor Jacob Tsimerman of the University of Toronto on receiving the Fields Medal — one of the highest honours in mathematics.
Nearly a century after Canadian mathematician J.C. Fields established this award in Toronto, it comes home.
100%. #SAVEICU trial used a pragmatic effectiveness methodology reflecting real-world sedation practice in ICU.

I’m don’t agree. I think #SAVEICU Trial shows 1. that setting trials in the noise of clinical practice is vital to understand the true effectiveness of an intervention. 2. Sedation with volatile agents is not better than intravenous sedation. Met-analysis shows it is not worse.

#CCR26 Last trial of the day, ABC post-ICU evaluating liberal RBC transfusion (aiming Hb 100-120) vs. restrictive (Hb 70-90).

➡️ Volatile anaesthetics cause bronchodilation & are anti-inflammatory
➡️ Could this sedation strategy translate into better ICU outcomes?
➡️ Or does the SESAR signal of harm in ARDS still matter?

Critical feedback is essential for clinical skill development but often fails due to mismatched delivery intensity and learner sensitivity. Saddawi-Konefka and Baker present a novel framework to explain these failures.

GA-CARES reminds us that biologic plausibility ≠ clinical benefit. Propofol-based anesthesia did not improve cancer-related outcomes after resection of aggressive malignancies. Evidence to guide practice.
