🫁 ⏰ When do you perform bronchodilator reversibility testing to help support a diagnosis of asthma?
We had a 👀 at the what effect time of day and season has on responsiveness @HLRI_Cambridge @CUH_NHS
Results published in @ThoraxBMJ today! thorax.bmj.com/content/early…
Do our body clocks affect how well diagnostic tests work?
Researchers @HLRI_Cambridge @CUH_NHS @RoyalPapworth have shown that a routine test for diagnosing asthma works better in the morning, becoming less reliable throughout the day.
The reliability of the spirometry test also differs significantly in winter compared to autumn.

A thread about consultants about giving feedback. 🧵
Consultants: Have you ever looked at feedback from rotating doctors - either end of rotation surveys or GMC survey data - that highlight a lack feedback in your department?
And thought ‘but I give feedback all the time?!’ 🤨

For surgical history nerds:
Here is the main figure from Seldinger's 1953 paper showing what later became known as the Seldinger technique.
There seem to be 2 extra things that Seldinger did that most people probably don't do these days, as we'll see next:

Physicians! You need to know the figures from this study when you talk about lumbar puncture with your headache patient.
A negative CT means a low post-test probability. How many LPs do we need to do to identify one SAH? What will the patient accept? @acutemedicine
The SHED study is published! Thanks to the 100s of contributors around the UK...
This paper raises a lot of questions - how will it influence your work? What post test probability of SAH and aneurysmal SAH are you, and your patients, comfortable with?

when I held family discussions on ICU, I always felt that being upfront—& even _blunt_—about a pt’s outlook/risk of dying was the kindest thing I could do.
giving false hope by omission, and denying loved ones the ability & time to say goodbye, might feel nice, but is so unkind.
A superb resource which helped me get through the respiratory SCE a few years ago, and now continues to be fun and educational. Learnt a lot about PH this week. Thank you!
