American medicine spends more money avoiding malpractice suits than it pays out in them. The doctor in this story won his case and left medicine anyway. Two-thirds of malpractice suits are thrown out before they reach court. Of the ones that do reach court, eight out of ten find for the physician. On paper that is a system working as designed. The verdict was never the point. Jordan Grumet, MD, an internal medicine physician, was home with his kids at eight at night when a process server rang the doorbell. The case was one he barely remembered, a patient he had a glancing role with who died of something nobody was going to stop. It took five years to resolve. Near the end, the lawyer essentially admitted he had never believed Jordan did anything wrong. He was using him as leverage against a nursing home. Jordan won. He also left clinical medicine. That is the part the dismissal statistics hide. The best study he could find put defensive medicine at roughly 55 billion dollars a year against about 40 billion actually paid out in claims. By age 55, he says, somewhere between half and 60 percent of physicians have been sued at least once, and the risk concentrates heavily in emergency medicine and the surgical fields. The dominant cost of malpractice litigation lands outside the courtroom, in what a physician does for the rest of a career to make sure the doorbell never rings again. Grumet's larger argument is that malpractice is one line item in a longer bill. Insurers, pharmaceutical companies, private equity, venture capital, electronic medical record vendors, malpractice attorneys. Every one of them sits between the two parties who actually generate and receive care, and every one of them takes something out on the way through. The line worth saving from the conversation is this one. When a business entity decides that visits run ten minutes instead of fifteen, or that a given test is too expensive to order, that entity is practicing medicine. And in Grumet's framing, business entities don't take the Hippocratic Oath. They carry a fiduciary duty to shareholders instead. We have handed clinical judgment to organizations whose binding legal obligation runs to someone who is not the patient. His conclusion is that there are only two real stakeholders in health care, the people who hold the knowledge and the people who need it, and that everyone else is a third party who has learned to bill for standing in the middle. Please Donate! and if you cant donate please share this post! Ladies and Gentlemen, It is that time of year again where I do my annual fundraiser to buy tickets for junior service members to attend the 2026 EOD Memorial Ball in May so that they can attend at no cost. last year, through this fundraiser, dozens of Soldiers were able to attend for free. This year, tickets will go to any branch of service, to junior members in the grade of E5 and below. The EOD Memorial Ball is put on by the EOD Warrior Foundation, a nonprofit that deeply supports the EOD Community. It is important to note that any money donated goes direct to the foundation. the money never touches my hands. Please give, and let's make this year better than the last! gofund.me/a506f4e7d

Actually the Medicare conversion factor is the boss of inflation. Hasn’t given physicians a pay raise for their work in nearly 34 yrs. 🤦🏽‍♂️😤 @AmerMedicalAssn ama-assn.org/system/files/cf…

Government healthcare is really a massive opportunity for regulatory capture. Medical Students need to be on X and get a real education! And it is a debt free experience.

Just a simple reminder today that Medicare thinks physician work is 30% less valuable today than it was in 2001. Despite asking us to see more patients, click more buttons and fill out more paperwork, we deserve to be reimbursed 30% less than we did 23 years ago. If you're considering a career in healthcare, what would you do if the US government considers your expertise to be devaluing at that rate? Medical students recognize this, which is why a recent survey found that over 60% don't plan on practicing clinical medicine. They want to pursue consulting or public health careers instead. I don't blame them. If you held any asset that was depreciating at that rate, you'd invest that capital elsewhere. We will never fix the physician fee schedule, though. Our only hope is to even the playing field. We must fight for site neutrality and allowing physician owned hospitals. We must reform 340B. We must fight certificate of need laws. Lets put delivery of healthcare in the hands of physicians. @DoctorJesseMD @sdixitmd @anish_koka @JPGK_MD @KatieOrrico ama-assn.org/practice-manage…

Multiplan is another scam; of course, it's owned by a PE firm. Insurers and Muliplan take a "fee" as a % of the amount of an OON bill that they "saved" the employer from paying. Granted... some OON docs charge rediculous fees... but that's not the point. The entire insurance market is a scam. nytimes.com/2024/04/07/us/he…

Not an really OR saying.... but words we all live by. “Every surgeon carries within himself/herself a small cemetery, where from time to time he/she goes to pray-a place of bitterness and regret, where he/she must look for an explanation for his/her failures” -Rene Leriche

Good illustration of a technique that ACS surgeons should be aware of, either to use themselves or when re-operating on such patients.
The Retro-ileal window is cool tool to have in the #CRS toolbox when you need a tad bit more length. #latenightcolon
