I had been waiting for something like this to drop!
It is clearly stated in the 10YP that the preference will be for 'alternative' rather than 'traditional' providers of education.
Cos traditional providers tend to like standards and exams, which NHSE thinks

you are right
in addition, NHSE are openly hostile to the GP Partnership Model which has traditionally trained GPVTs docs
Even though it is the most productive and efficient part of the whole health system
Their hostility to traditional general practice is pathological

The reason A&E is rammed in #Nottingham is because many hospital out patient depts (Consultants & managers) have stopped seeing patients referred by their GPs
These pts get worse in the community and get bounced into hosp via a&e

I work in social care, I remember shouting from the rooftops about patients being discharged from hospital back in to their care setting without proper barrier nursing. I was told I was wrong at the time and the focus was to support the NHS by preventing bed blocking in hospital.

Basically...what the powers that be are saying is this.
'We can't afford the NHS. And to sustain it, you, staff, need to take a below inflation pay-rise aka a cut.'
Now- to improve work productivity, you need a couple of other things- beyond pay.
Good working conditions, less discrimination
The NHS can't offer any of the above.
And wonders why the NHS isn't productive.
Workforce planning is an ethereal thing in the UK health care system.
So we are where we are.

Huge concerns about the lack of declaration of conflict of interest around authors/contributors of @lengreview protocol -continues.
Just sitting down to do the response to @RCPhysicians as a Fellow
And spotted this gem in the document
A PA to do peritoneal drainage, PICC lines, Ascitic drains, Biopsies.
And a Lumbar Puncture.
In an environment where @RCPhysicians has just published a document on Resident Doctors
I have 2 questions to all leaders -still driving this.
1) Why would you encourage such unsafe stuff on the population
2) Have you all lost your mind
No way, any of my loved ones would be getting an LP from a PA- why are you doing this to others?
Insane stuff.

I keep hearing the line of “doctors make mistakes- so it’s ok for other staff to be doing their jobs too”
A few things.
1. Aviation is touted as an exemplar of safety. When a pilot makes a mistake- the industry works to give them added training, support, breaks as needed - along with learnings to prevent any further such
2. What it doesn’t do is ask air traffic controllers to fly the plane. As they realise the safety issues will become more, not less.
Or for that matter- the baggage handlers
3. Playing footsie with safety issues is what has led to the NHS being so poor on safety outcomes.
Lower skilled people doing work outside their competence with light touch regulation is not the only reason- but is contributory. If you don’t understand that due to your hubris? I can’t help you.
I don’t work as a GP, Physiotherapist, Pharmacist or a Nurse for a reason
It’s called awareness of one’s limits and risk of being unsafe to others health.
It’s simple when you think of it.
Play as much mental gymnastic with the above as you want
But people are waking up to it all around.
Stop chicanery, clownery and charlatanism.

GP thrown under the bus. Why not the ICB Medical Director? Or the ICB prescribing lead?
Where is the Royal College? Why aren't they saying that this is unacceptable.
Anyone seen the LMC? Or even the GPC?
We have no one. Absolutely no one in our corner.

Here is another example. The GP is following the local ICB prescribing guidance but it is the GP and practice that will have to respond to the complaint, cope with the stress and deal with the fallout if the complaint is taken further. bbc.co.uk/news/articles/c6vg…

Corridor Care to end by the end of 2029 - so potentially 50,000 - 90,000 deaths between now and then, avoidable
Well done @YvetteCooperMP @andyburnham 👏🏼👏🏼
Just for comparison 160 ‘plane crashes’, over half the BA fleet
Honoured to speak at RCEM South East Coast conference
Research estimates 554 excess deaths a week linked to ED overcrowding.
Government aims to end corridor care by 2029.
If this were aviation, would we accept waiting three years?
We've normalised the unacceptable. #NHS

In a surprise to absolutely nobody, the forever solution to “save money” by moving work out of hospitals to “neighbourhoods” (née GPs) results in the fascinating equation of:
GPs do more work = hospitals continue to receive resource for that work
And if we say no…
As NHSE's Neighbourhood lead departs, NHSE admits it is short of money for implementing "neighbourhoods" (framework announced March 2026), as not only is there no new money, but there are deficits

It's increasingly hard to do a good job.
Doing the right thing creates complaints, longer appointments and prescribing less.
Make a decision yourself? Nah, use A&G.
Absorb the risk? Nah, refer everyone who meets 2WW criteria.
Watch&wait? Nah, initiate the sepsis bundle.

Truly shocking culture war on #BBCQT
Former Conservative now Reform UK's Suella Braverman who was born in London says,
"I'm not an English person"
Then suggests you can only be English if you have a lineage or ancestry which dates back a few generations.
A black man in the audience says he considers himself English because he was born here, and that she is enforcing a racist view that says being born here does not make you English.
Oli Dugmore, "The political function of what she was saying earlier is that you can only be English if you are white.. That is a racist point of view"
Fiona Bruce then repeatedly asks Suella Braverman to address Oli Dugmore's allegations, she repeatedly refuses to adding, "I do not call myself English, I am British Asian"
At the end, Freudian slip, "I don't consider myself to be white, er er er English. I'm Asian of Asian heritage. I don't see why that's controversial"
My view: Braverman is entitled to describe her own identity as British rather than English.
But turning that into a definition of who can be English is racial exclusion dressed up as patriotism.
A Black or Asian person born and raised here, speaking with the local accent, sharing the life and culture of this country, is no less English than anyone else.
England has never been a racial category. It is a country, a culture and a shared civic identity, and trying to make whiteness the entry requirement is both historically illiterate and deeply divisive.
Braverman has previously argued that people without generational ties to England should not claim to be English.

I'm not sure why she keeps repeating this line that she is being accused of "being racist against myself" as if it's some sort of mic drop.
She literally is being racist against herself and anyone else who looks like her, has her ethnic/immigrant background. To say you have to be white to be English is a totally racist statement.
And this cop-out that she's British not English doesn't deal with Italy or France, where they don't have two identities. So is she saying brown and black people can't be French? If so, then what are brown and black people in France? Or Italy?
The idea that the England football team captain can be Black but Braverman says he won't be considered English is both absurd and, yes, racist.
And her being brown doesn't give her a pass or cover for her racism, it just means she's a self-hating racist.
You’ve called me racist this evening- albeit in a contorted way- but in doing so, you’ve lowered the tone of the debate with divisive and inflammatory language.
And absurdly, you’re suggesting that I’m being racist against myself.
It’s because of irresponsible language like yours that we in Reform need security.
The left can keep smearing us as racist for challenging the cosy consensus on mass migration but we will not be bullied into silence.

A patient received a referral rejection letter after she'd already had an appointment!!
They'd rejected our referral, so we rejected their rejection & they then accepted the referral, but by that time they'd already sent out a rejection letter!
