It is 2026 and I still see these routinely stocked in ERs and ambulances.
Here's to a worldwide ban on the "tonsil tip/yankauer" from every emergency department and ambulance.
It has absolutely NO place in urgent airway management. Arguments?

Someone on X (often anonymous) posts an extraordinary medical claim (ex. HIV doesnโt exist). When asked for specific objective verifiable evidence to support the claim you get told to do your own research & hereโs a website. Often it comes with ad hominem attacks :(

Intubation + severe metabolic acidosis = high risk.
Lose respiratory compensation, even briefly, and patients can crash fast.
Know when to hold off, how to prep, and how to ventilate if you must tube.

The whole truth about whole blood?
We still need more LTOWB studies, timing matters, logistics matter, and the battlefield continues to answer questions.

๐๐๐๐ ๐๐จ๐๐ฌ๐ง'๐ญ ๐ฐ๐จ๐ซ๐ค!
That's what a new observational study from New Zealand reports.
That's where most people stop reading, and say "I told you so!"
โ But let's not make that mistake.
A result like this begs the question, "How does this make sense when the DOSE VF RCT showed that DSED more than doubled survival in refractory VF?"
This is why the headline read of a study is rarely the read that matters.
โ
The methodology is.
โ
The patient population is.
โ
The timing of the intervention is.
And the limitations the authors themselves disclose, often tell you more than the conclusion.
I took a deep dive.

SOHO trial in NEJM:
๐๏ธ Multicenter RCT comparing high-flow nasal cannula vs. low-flow oxygen among patients with acute hypoxemic respiratory failure (mostly pneumonia)
๐๏ธ Inclusion criteria required a respiratory rate >25 and substantial hypoxemia (P/F <200)
๐๏ธ No difference in the primary endpoint of mortality (not surprising, the mortality endpoint is nearly always neutral in modern RCTs evaluating the nuances of supportive care).
๐๏ธ HFNC reduced the rate of intubation and increased the number of ventilator-free days.
๐๏ธ No significant signals of harm from HFNC.
This is similar to the FLORALI trial (by the same group of investigators). However, FLORALI showed *improvement* in mortality, whereas the current study didn't.
Why do these studies disagree about mortality?
The FLORALI trial found no mortality difference between HFNC versus conventional oxygen among ALL PATIENTS. However, it DID find a mortality difference in the *subgroup* of patients with P/F <200.
So this is a classic tale in critical care - a subgroup analysis from one trial subsequently fails to replicate. (At least, in terms of the mortality benefit.)
But chasing mortality in these trials is a fool's errand. Overall, the SOHO trial still supports the use of HFNC in acute hypoxemic respiratory failure (in terms of reduced intubation and improved ventilator-free days).
Using HFNC for sick pneumonia patients is already standard care, so I don't see this changing practice very much. Keep calm and HFNC on. ๐ฌ๏ธ

From the 2025 FAA data regarding the HEMS industry:
Only 49% of flight requests result in a patient transport. ( the balance were missed due to weather, maintenance, aircraft availability etc)
Nationwide average of the 385370 patients transported were 30% scene, 70% interhospital
The average program flew 285 pts per ship per year (0.78 pts per 24 hrs)
Only 2.3% of the total HEMS hours flown in 2025 were under IFR ( vs 100% IFR for US passenger aviation)
Fatal accident rate (per 100k flight hours) has increased 46% from 0.39 in 2024 to 0.57
