I have never understood why we do need non-inferiority trials. In my view, they are a great invention of pharmaceutical industry to introduce me-too drugs without demonstrating any real advantage with low cost trials. This excellent review fully explains the key concepts 👇👇👇👇
You don’t understand non-inferiority trials (and neither do I) buff.ly/3MfdgOX

And although I cannot say this for a fact, I would be surprised if the NI margin were not defined at study start. This was an investigator-initiated HTA trial.

Actually, this is now me being sloppy as well. If you follow me down the rabbit hole: fundingawards.nihr.ac.uk/awa… njl-admin.nihr.ac.uk/documen… Dating back to October 2017. I hope this settles it.

The only solution is to move admitted patients out of the ED so we have the space and resources to get to the patients in a timely way. Give us a chance to save them @CAEP_Docs

Thank you Avis for sharing this case. A reminder that a) long wait times kill, b) perfect triage is impossible, and that c) it's not just patients who have minor issues who leave without being seen. 1/2

Our premier @fordnation once promised to end hallway care in Ontario Instead, under his leadership, our healthcare system is in the worst shape it has ever been I am forced to see ICU level patients in hallways in GTA hospitals, while he wastes endless money on spas and beer

Interesting, yea, 5% would seemingly be a more natural choice compared to 5.4%.

It's bugging me because if they had specified a margin of 5%, which is what i would have done just to pick a whole number, then the PCT-guided strategy would have been inferior to standard care:

The unusual precision of this margin is really bugging me. I don't mean to imply suspicion about it being determined post-hoc; 5.4% is in the protocol in the supplement, but unfortunately no margin is specified in the ISRCTN protocol. Heres the relevant text from the protocol

Yea, agree - does not make sense to me. Also, odd that there is no discussion elsewhere in the manuscript & conclusion throws in the word "safe" without any qualifier or even mention of a potential issue.

I agree. The federal government has tried to make this happen in the past and it is opposed by provinces who essentially say ‘Health care is provincial. Keep you nose out of it’

If we had nat’l licenses it’d be so easy for docs to help out anywhere. “The further away from a city you get the better the medicine is. The job is just so much more fun. […] I do think rural locums are key to staying a really good clinician.” @First10EM on @broomedocs podcast
I am amazed by so many of my Toronto medical colleagues regularly working in Timmons, Lake Huron's north shore, Sioux Lookout, Yellowknife and Iqaluit. We are such a vast nation. We are Canada's publicly funded healthcare system.
