NEW STUDY: Diet in One of the Deadliest Cancers.
I have studied ketogenic diet in cancer, and in particular glioblastoma (GBM) going back to 2014. GBM is arguably one, if not the most, deadly cancer we know of.
It has a 3-year survival ~10% with a median survival of 12-18months with standard of care treatments.
When we were studying these diets in GBM, the mechanistic rationale, growing preclinical evidence...but we had very little human evidence (only case reports).
Awe-inspiring to see this work by @drmclphillips: doi.org/10.1093/noajnl/vdag1β¦.
RESULTS:
1) 3-Year Survival:
-Standard of Care: 7%
-Ketogenic Metabolic Therapy: 27% π€―
2) Median overall survival:
-Standard of Care: 14.7 months
-Ketogenic Metabolic Therapy: 21.5 months π
....that is absolutely incredible!!!!!!
3) 83% sustained the metabolic therapy, targeting a specific glucose:ketone index (called GKI). β
4) Side effects of the intervention were mild to moderate, showing it is well tolerated.π€
5) Exercise and quality of life improved....πͺ
LIMITATIONS: π€
1) Not a randomized, blinded, placebo-controlled trial.
2) Intervention was not just ketogenic diet, but also intermittent fasting and exercise (comprehensive metabolic intervention) alongside chemoradiation therapy. A complete lifestyle therapy.
3) 32 participants is actually a huge number for a trial like this...but not HUGE in the grand scheme of pharmacological clinical trials.
TAKE HOME MESSAGE:
1) Diet is not a replacement for cancer treatment. π½οΈvsπ
2) But metabolism is becoming a critical part of cancer treatment, as multiple lines of evidence have shown across other cancer environments in humans (e.g., ketogenic diet in GBM; fasting in breast cancer; etc.)

π¨π¨π¨ Breaking study in #anorexia research
A supervised #keto diet helped 72% of women (13 of 18) drop below clinical thresholds for both anorexia AND depression in just 14 weeks β while keeping BMI stable and dropping anxiety & food compulsions. newscientist.com/article/252β¦
Years ago, I learned of a published case study describing how a ketogenic diet helped @Carobeckwith recover from a 15-year battle with anorexia. Today's newly published research is a direct result of Caroline and her family's valiant journey, and their determination to help others explore the metabolic intervention that put Caroline's anorexia into remission.
At @BaszuckiGroup we've been honored to support @GuidoFrank and his team who designed, launched, recruited for, completed and published this pilot trial of ketogenic therapy in weight-normalized anorexia nervosa.
This paper is the culmination of many years of hard work--including from those who participated in the trial. A second study is already underway.
One family's journey sparked one clinician-scientist to want to learn more. Together, Dr. Frank and the Scolnick family have challenged the field to explore the metabolic roots and treatment possibilities of a disorder in which current treatments fail far too many.
Please share widely. A second nation-wide trial is enrolling now.

π¨New science DEBATE released on HIGH CARB vs KETO: What the new @ProfTimNoakes vs @LouiseMBurke debate actually shows π
American Journal of Clinical Nutrition just released it big debate between two of the biggest names in exercise performance asking one loaded question: does a low-carbohydrate diet impair endurance performance?
On one side is @LouiseMBurke, one of the most influential sports nutrition researchers in the world. Her argument: yes, low carb can impair performance? She is not saying every athlete must eat high-carb all the time. She is saying performance depends on the event, intensity, environment, training status, sex, and the athlete in front of you.
ajcn.nutrition.org/article/Sβ¦
Her biggest physiological point is fuel economy. Near an athleteβs oxidative ceiling (i.e., elite race pace), she argues that carbohydrate produces more usable energy per liter of oxygen than fat. So even if keto adaptation dramatically raises fat oxidation, that does not automatically mean better speed or power.
Now I am skeptical of this point, since we have conducted Randomized Controlled Trails in both runners and ironman competitors and showed that athletes at 86% of their VO2max can utilize fat as the predominant fuel while on a very low carb diet and STILL maintain performance.
pubmed.ncbi.nlm.nih.gov/3684β¦
On the other side, @ProfTimNoakes argues: low carb does not necessarily impair performance when athletes are adapted long enough. He points out that many βketo hurts performanceβ studies are short, often under four weeks, and that several randomized trials lasting four to six weeks report similar performance between high-carb and low-carb groups.
ajcn.nutrition.org/article/Sβ¦
He also reframes fatigue. Instead of saying muscle glycogen is always the main limiter, he argues that during prolonged exercise, maintaining blood glucose, the small glucose pool is the most important. In our trial, 10 grams of carbohydrate per hour improved prolonged cycling performance after both diets.
Noakes and Burke also did something really special: they developed a CONSENSUS article where they explored key points of AGREEMENT and DISAGREEMENT for future research. This is the coolest part and in my opinion was one of the verty unique aspect of these three studies
ajcn.nutrition.org/article/Sβ¦
My take is this: It is not "team high carb" versus "team keto." The weight of the evidence demonstrates that
1) BOTH high and low carb diets can work, and that
2) Carbohydrates during prolonged exercise are valuable...but
3) How this is applied depends on the individual athlete.
What's your take?

Perfection is not required. (However you even define "perfection" anyway.) What really moves the needle is *consistency* applied over time. Small things, repeated over & over, over a long period of time.
