Measure twice and cut once 🔂! Retrograde access and one-burn ❤️🔥 elimination of this bigeminal PVC from superior mitral annulus. Notice the CS activation bracketing at CS 5,6 which was just across from the earliest endo location. Case performed by "One-burn" @Maeskander. #EPeeps

Hey #EPEEPS How's this for a circuit?!?
Atypical Afl with a long narrow channel in a very low voltage area.
EGMs spanning the full TCL were imperceptible on the recording system, but not on Grid with Nearfield-OT.
Frequency based mapping annotates the critical isthmus with almost-perfect accuracy.
Less than a handful of bad points in an 18k point map, voltage cutoffs of 0.1 mV would've hid the majority of this isthmus on the map. The propagation map below has voltage cutoffs of 0.02 mV.
Voltage, however, is still useful in conjunction with frequency mapping. The whole channel lights up with low voltage, high frequency emphasis giving us a discreet target.
Flutter termination when putting the catheter on the channel. AML to follow.

First time mapping a Farapulse PFA case using the new Ensite X v3.0.2 Enhanced FaraWave visualization software. Being able to visualize the catheter in its true basket/flower (🧺/🌺) form on the mapping system is a game changer! #EPeeps #PFAonX Case performed by @Maeskander.

Fluoroless PFA #Farapulse pearls (after achieving optimal safety and efficacy):
1) ICE guidance key for contact, left sided ICE especially for right sided veins.
2) Wire tracking.
3) Mapping visualization of catheter confirmation.
4) Projection mapping.
Even for PFA, #MappingMatters!
With Ensite X, visualize your preferred PFA catheters and wire, assess tools proper positioning and track PFA applications via a PF projection map.
Here's how well these maps correlate with their post voltage maps!

I haven't always turned on fractionation dots in the past, but it does seem to come in quite handy sometimes to highlight mapping points/areas with higher fractionation. #SandrineDots @EP_SandrineDM

Last case of the year and redo AAFL. Patient had prior Maze, PVI + CTI line, and most recently a plethora of LA flutters. Went straight transseptal this time, thinking it had to be a recurrent LA circuit, but after extensive mapping, we found we were in the wrong chamber. #EPeeps
