This may be the dividing line separating skilled and unskilled therapists. Unskilled therapists see pain and victimhood. Skilled therapists also see the hate and aggression and know how work with it. How do we know there is hate and aggression? Because the patient is human. 👇
“It is one thing to empathize with patients’ experiences of injury and victimization but we must be able to empathize with their negative impulses as well, including sadistic or hateful wishes to attack or to wreak vengeance.” —M. Carsky & F. Yeomans, “Overwhelming Patients and Overwhelmed Therapists”

🚨 NEW: For the @WSJ, @Hartz_PhD and I expose the ideological takeover of the American Psychological Association. The APA is facing the possible loss of its federal accreditation authority following allegations of widespread antisemitism. But while the APA's antisemitism problem is serious, it's only a symptom of a much deeper ideological rot. The organization has become so captured by radical left-wing political activism that its commitment to scientific objectivity competence is now in serious doubt. We examined the program for this year's major APA Division 39 conference, which drew nearly 800 attendees and held 90 panels over four days. By our count, roughly 70 of the 90 panels were overtly political, and always left-wing. Topics included abolishing the family, queer anarchist psychoanalysis, dismantling gender assessment guidelines, and white supremacy. One session even concluded with a drag show. There was at least some hope that the threat of losing federal accreditation might prompt some institutional soul-searching. It didn't. Instead, APA members just elected Candice Hargons as their next president. Her campaign identifies "systemic intersectional oppression" as the profession's "common enemy" and calls for rejecting the traditional scientific notions of objectivity. Psychologists wield enormous influence over vulnerable patients, and the people seeking treatment deserve the peace of mind knowing that the care they're receiving care is evidence based and not tainted radical politics. We believe most would agree that federal accreditation should actually mean something. If the APA cannot distinguish its scientific mission from its political activism, the Education Department should reconsider whether it deserves the government's endorsement as an authority over psychological training. From the high schools of France to the streets of Madrid to the universities of Tirana, young people are telling their governments the same thing: in an age of massive wealth inequality, they want a future they can believe in. In France, students and teachers are asking for safe schools, enough teachers and smaller classes. In Spain, for affordable rent and housing. In Albania, for good jobs and an end to corruption. @JDVance says this is what happens when a nation lets in too many immigrants from the global south. He is not just morally wrong, but misreading the moment. The is what happens when nations let wealth and power accumulate in the hands of a few. This is what happens when young people lose hope in their future. In America, let us listen to our young. We need an economic and political transformation. America's Next New Deal.

The obsessive cannot be helped unless the therapist is willing not only to tolerate but to actually sustain the risk of therapy ending prematurely. Without this risk, the therapist allows themselves to become a relic permanently on display in the obsessive's psychic museum as a dead object only to be preserved. The therapist must refuse this. They must establish themselves as an object not of the dead, but of the living and breathing. Some will flee, but if the therapist cannot tolerate losing the obsessive, the obsessive will never learn to bear the existence of a living other that comes and goes according to its own whims outside of their control. Nor will they learn to bear the other's eventual death and decay that renders their efforts of preservation totally impotent.

Different cultures amplify different kinds of psychopathology. If the 1980s were the “Culture of Narcissism” (Christopher Lasch), the 2020s are the culture of splitting and projection. It just doesn’t lend itself to a pithy book title. 👇
1/ One of most important things I've learned: Severe personality problems find *camouflage.* No one thinks “I’m a sadist” or “I'm a malignant narcissist.” They find a belief system/social group that validates their most hateful, destructive impulses & construes them as virtues.

1. Most therapists misunderstand or do not understand: transference, countertransference, defenses, and their own baggage. 2. Most therapy isn’t helpful because of the above. 3. Most therapy could be briefer if they truly understood point 1.

If any billionaire wants to give me about $50 million I can create a low relapse rate, humane, minimal medication, inpatient long term care program.
There is a tremendous need for inpatient psychiatric care. Patients & families are desperate for therapeutic inpatient treatment with psychological support that offers them dignity, privacy & respect, but what they often experience instead is traumatizing & dehumanizing. (1/3)
