Prescription drug insurance/PBMs may also play a role in decisionmaking for patients & doctors. 3 PBMs (CVS Caremark, Optum/United, Express Scripts/Cigna) control around 80% of all US prescriptions and they use formularies to list which drugs each PBM covers & the tier levels.. A prescription drug insurance company is able to refuse to cover one (or both) of the GLP-1 brands on their formulary for any reason (often it’s financial not for medical reasons). And if it’s a lot cheaper for the patient to get the drug listed on the formulary , they’ll usually the choose the drug the PBM “recommended.” This is why many type 1 diabetes patients found themselves in a position where they had to switch insulins at random times for no medical reason—because their PBM/prescription drug insurance decided to drop the insulin they were taking from the formulary. Some states do more to regulate this than others.

I finally listened to my own advice.Lying on that colonoscopy table not as Dr. Yüksel, but just as me, a person who’s scared of what might be found, my heart was pounding. What if something was wrong? What if I’d waited too long?
Then came the moment: the screen showed a polyp. Risky. Sneaky. The kind that can sit quietly for years before turning into something far more serious.

Perspective @NatureCancer on GLP-1 medicines and cancer risk. Reducing obesity can reduce cancer risk, but beyond that, GLP-1 receptors are expressed on certain cancer cells - could be good or bad. Real world studies suggest lower incidence of some cancers. Overall, impact on inflammation, TME, and cancer risk needs closer study.
