Laryngeal carcinosarcoma (sarcomatoid SCC). Sometimes you have to look v hard to find any evidence of epithelial differentiation. This was the only 1mm focus +ve for p40 and cytokeratins. Don’t believe it’s a sarcoma until you’ve done levels and every CK in your armoury #ENTPath

A giant of cancer pathology drawing a line in the sand. My view...unless you can show real impact on patient outcomes, much of this incessant splitting
and updating is self aggrandizement by those who wish to reinforce their position as courtiers in the ivory tower.

This weekend in A&E, I saw patients who couldn't get GP appointments
And I worked with GPs who couldn't get jobs as GPs
@NHSEngland make it make sense please as i can't.

Love my new book!
Of course chapter 14 is the best bit 😉
Recommended for all histopathology/forensic pathology/neuropathology trainees, as well as practising consultants. A wealth of autopsy knowledge from fabulous authors.

Merry Christmas to all those working in the NHS
Many staff in the NHS are grossly underpaid (even on Christmas day👇) but none more so than Resident Doctors who frequently graduate with >£100k debt
RT to support them & ask government to pay them ALL fairly #PayRestoration

Atlas of Adult Autopsy second edition (Kim Suvarna & Abi Sharp Eds) is now published! With a brand new chapter on autopsy histology (by me 😊).

Microscopic colitis with giant cells. Female with autoimmune history and watery diarrhoea, negative micro and endoscopy normal. Unlike CD which has deep granulomas the giant cells in MCwGC are superficial and dispersed. Like CC and LC responds to budesonide. @drkeithsiau #GIPath

Perhaps I’ve had too much coffee but the forceps artefact in this skin biopsy shared with me by @ssaunders279 reminded me of the clashing tusks of a pair of rutting narwhals #pathart #dermpath @ACP_Pathologist

Clear cell metaplasia in a sigmoid polyp from my colleague @edinpath. Resembles secretory endometrium and perhaps Sertoli cells! No clinical importance but pretty! I was a doctor.
I lived and breathed medicine.
Then burnout & mental illness happened.
Upset & angry, I left medicine.
If you care about the #NHS,
And you plan to vote in the #GeneralElection
You NEED to know these 3 things.. 🧵

Sometimes IHC is not needed but you do it anyway because pictures speak louder than words. Oesophageal biopsy from an elderly man. Right panel is HSV-IHC.

Wow! Dr. @Andres_Matoso teachings on MMR IHC interpretation (Note: you can replace the 2nd floating person w/🪁 kite or 🎈). MLH1 and PMS2 hold hands (heterodimer). If MLH1 is lost, PMS2 will also be lost. If PMS2 is lost, MLH1 remains. @Blake_A_Johnson #Surgpath #Pathtwitter

Look at all the medications in this tiny portion of a gastric biopsy. The drugs present tell us that the patient has renal disease. Of course, microcrystalline cellulose is just pill filler. Kayexalate reduces hyperkalemia and Renvela reduces hyperphosphatemia. #UMiamiPath

Today I made a mistake. A diagnostic inaccuracy. The culprit: me. The cause: excessive pressure from the oncologist. I'd like to think it won't happen again, but... Let the pathologists do their job and not demand answers in a hurry.
