Is a plastics opinion warranted for this?

Errors are inevitable in medicine When I think back, I would attribute most of the ones I've encountered to a lack of information This is usually in the form of some crucial piece of information That 'last known normal' time wasn't correct The patient with 'new right facial weakness' actually had a stroke with residual weakness 2 years ago The patient who is 'hearing voices' actually has very poor sleep quality and increased caregiver related stress who's hearing her husband's voice as she falls asleep. She also has a history of sleep walking/talking So much of what we need to know to care for our patients comes from talking with them and those who care for them I contrast that with some days at work where there will be a census of 30+ follow up patients to see and 10-25 new consults I have a team, it's not just me But it is a lot of info to juggle all the same As we see shortages of all physicians looming, not just neurologists, these longer conversations that lead us to the root cause of someone's problem are becoming more and more dear My suspicion is this will lead to more unnecessary tests, wasted money, and, ultimately, wasted time One of the things that drew me to neurology is that so much of the information needed to make the diagnosis is right there in the room with you and your patient Hearing their story Observing them, examining them One of my favorite, perhaps apocryphal, quotes of Osler is “Listen to the patient; he is telling you the diagnosis." While he may not have actually said it the sentiment resonates with me and I've found it has proven true time after time in my own practice I don't have any answers These are just musings that I have as I review my list and drink my coffee in the morning I do believe that education remains an essential part of addressing these issues And when something doesn't make sense, go back to your history Take the time now It will save you, and your patient, time in the future #medsocial30

I am not normally one to post. However - there is a limit to how many winters one can tolerate on a late summer’s day. If only I could have a mug that reads FUBAR

A thread on why the UKFPO’s decision to randomly send newly qualified doctors all over the country actually makes political and financial sense (Disclaimer: I have no evidence and this is merely my own rambling opinion)

So many issues with PAs following F1/F2 and on to GP / specialist training If their training is the same they should to take MB BS to prove that F1/F2 shouldn’t be used as a test of capability - it’s training Medical schools and Royal Colleges are very particular about their curricula and standards. Have they been getting it wrong all this time? What we can all see is this government creating a medical workforce of non-doctors whom they can train quickly with qualifications that won’t be recognised overseas. Until these issues are resolved doctors, @TheBMA and Royal Colleges should not support these non-doctor programs @BMA_JuniorDocs @TheDA_UK @dr_barrett @parthaskar @trishgreenhalgh @djnicholl @TomStocks1982 @Parody_RCGP @DrSteveTaylor @goldstone_tony @Existential_Doc @drmattuk @DrAmirKhanGP @mmamas1973 @DrEilidhMaria @EmergMedDr @dr_irfan_malik @ShafiAhmed5

I am a NHS consultant. I am financially comfortable, in a job I love, so why am I striking tomorrow? Answer: Because the next generation will not be able to live off what I do today. The juniors that will become these consultants know this and are leaving already. Why? 🧵 👇

THREAD on #Consultants #Strike: As a consultant oncologist, this is how I am going to support the strike: I will work tomorrow because I have 2 MDTs, which can’t be cancelled because it would delay cancer treatment decisions by a week for about 80 patients. 1/

“Twice this week, I have watched an elderly individual, fade into the busy life in which we all live. One man just needed Panadol for his wife but the shop assistant simply said it’s in aisle ‘6’. But he struggled to navigate the supermarket and as I watched him go in the wrong direction, I left all my groceries and took him where he needed to go. Today, I watched an elderly man struggle in the heat, who had obviously had a fall with a huge scrape and blood on his leg. He walked past people in the cafe, while he slowly made his way to his car. Not one person stopped. Or looked. Or acknowledged him. I took him to his car and checked he was ok. He told me he had a fall and wasn’t sure how the air con worked in his car so he just didn’t use it. I sat with him, until his air con kicked in and heard him talk about the old frail body that he is in, that fails him now, every single day. When you see an elderly person walking down the street, searching in the supermarket or struggling to their car, take a minute out of your busy schedule and ask them if they need a hand. Think about your grand parents and your parents and how pissed you would be if someone didn’t stop to help them. But more, think of them as you. Once upon a time they were you. They were busy, they had work, they had children, they were able.... Today, they are just in an older body that is not going as fast as it used to and this busy life is confusing. They deserve our utmost respect and consideration. One day it will be you, it will be us. I wish more people gave a sh*t about them and acknowledged them for their admirable existence and geez I hope someday, not that far away, someone does it for me.” ~Adele Barbaro

So frustrated by #taxfreechildcare 🤯 I’ve had to set up the standing orders with a £0.01 difference to be able to pay into 2 accounts on the same date. Otherwise the SO instruction didn’t recognise 2 reference numbers going to the same a/c and s/c listed as 2 separate payees

“…and we used to have prescribe all their doses for the weekend and chase an INR on the Sunday if we were the F1 on call and write their discharge doses in a little yellow book and we’d never even heard of a NOAC!’

When part-time is really full-time When GP hours are counted Official figures for GP working hours are counted in sessions A session is 4hrs 10 mins or half a day 9 sessions is 37.5hrs or full-time officially But there is a problem 🧵1

A few myths to dispel: 1. Teachers are not paid for the holidays. You read that right, teachers are not paid for the holidays. They could be classed as TOIL for the unpaid overtime they routinely do, but actually they are just not paid (despite many working, prepping etc.).

It’s more than language. These archaic mindsets need to change and have no place in our sport. I’ve been surrounded by these attitudes and targeted my whole life. There has been plenty of time to learn. Time has come for action.
