🔴Teaching point on ERCP #Basics
1️⃣If the papilla is floppy; you get in but you are not going anywhere ( Pushing will Not Work ); it's about the correct angle; try to tip up or at least align in the same biliary access ...
2️⃣other points once you unintentionally went inside the P.D. Every time you go in you dilate the PD entrance and it's become harder to axis the biliary system...you need to have a clear strategy...
3️⃣For DGW techniques; it's helpful to use 0.025 as 2nd wire as it's flexible and will find the way...

#ERCP can be challenging due to #roux_en_y anatomy. As a result, #EDGE techniques are used. In this patient we used the #axios stent by @bsc_endoscopy to create a gastro-gastric anastomosis. This will be a 2 step procedure. Only had a 10x10 that day but made do. #gitwitter

I would have used a proctoscope to protect the anal canal from getting cut while removing the blade with the rat tooth forceps. (Bring the blade until rectum, drop it there, insert proctoscope, put scope through proctoscope, bring the blade out with forceps or snare).
