I think it’s quite possible the faculty had even more fun than the fellows at the @ASGEendoscopy 2nd & 3rd Year Fellows Course. Such a great weekend getting to teach, learn, reconnect with old friends, and make some new ones, including a brand-new text group that I have a feeling will live well beyond this weekend. The hands-on stations were incredibly high yield, with practical tips on everything from hemostasis and closure techniques to PEGs and ergonomics. I even got my own personal ergonomics assessment… and apparently I have some work to do. Grip strength & posture need some work 🫣 But some of the best learning happened outside the formal sessions - trading GI tips and tricks with colleagues, talking career development and finances, comparing how we approach cases, and picking up those little pearls you only get when you put a group of people who love teaching and endoscopy in the same room. I came to teach and left with new friends, new GI pearls, new financial tips, a few things to change in my own endoscopy practice… and a reminder that being faculty doesn’t mean you stop being a learner. Every time I listen to a lecture on Electrocautery I learn something new. This year at @ASGEendoscopy FYF course @EndoscopyOthman & @HilarHolzy taught us the basics of ⚡️ Some high yield concepts: ⭐️ The pedal doesn’t determine the tissue effect. ⭐️ The selected generator mode primarily shapes the tissue effect by controlling the electrical waveform. ⭐️ The device then influences how that energy interacts with tissue through its design, contact, and current density. Think of it this way: ✂️ Cut → Rapid tissue heating → Intracellular water vaporizes → Tissue divides 🩸 Coagulation → Slower tissue heating → Protein denaturation, desiccation, and vessel sealing Once you understand this foundation, electrosurgical settings become much less about memorization and much more about understanding the desired tissue effect. One of my favorite traditions each year is returning to the @ASGEendoscopy First-Year Fellows Course. It’s always a privilege to teach, but it’s even more meaningful when our own fellows are sitting in the audience. Watching them take their first steps into GI fellowship while having the opportunity to be a small part of that journey is incredibly rewarding. Congratulations to @ASGEendoscopy & the outstanding course directors @TomTielleman @rmulkimd for an exceptional program. These courses don’t happen without countless hours of work behind the scenes. Fellows - as you’re learning today, here’s one career goal you may not have imagined yet. One day, you may be back, not as a learner, but as faculty. Standing beside colleagues who have become mentors, collaborators, and friends. Teaching the next generation. Those opportunities often begin long before you realize they’re possible. Enjoy every moment. Your future in GI is just getting started.
