🫀 Don’t stop spironolactone just because eGFR dips. TOPCAT Americas analysis (1,648 HFpEF patients): • 26% had early acute eGFR decrease ≥15% — and it was adversely prognostic • Early eGFR dip → HR 3.53 for sustained renal decline/ESKD/renal death • Yet spironolactone consistently reduced CV death, HF hospitalization & aborted cardiac arrest vs. placebo (P-interaction = 0.64) The CV benefit persisted regardless of early eGFR change. An acute eGFR dip after MRA initiation should not automatically trigger discontinuation. #HFpEF #Cardiology #HeartFailure #MedTwitter
