“Half the patients I see in the ER could have been treated a week earlier for a fraction of the cost.” Dr. @khakrish is building @CounselHealth around a different vision for AI: not replacing physicians, but supporting them. Rishi joins Offcall co-founder Dr. @grahamwalker to unpack “semi-autonomous care,” and what it means for physicians. 🔥 Full episode: offcall.com/learn/podcast/se… #HowIDoctor #HealthcareAI #Offcall

@CounselHealth is a full-stack, AI-native medical group that makes medical care abundant - unlimited guidance from real doctors, 24/7, scaled by Counsel’s medical AI, in a now familiar chat interface. This system embodies the qualities that everyone should expect from "AI as a New Site of Care", which go far beyond the simplistic, early models of digital health that simply replicated offline visits on a computer screen - e.g. care that is always on, with pan-specialty intelligence, personalized, and exponentially scalable. Thrilled to have led the Seed and be co-leading the Series A with our friends at @GVteam as Counsel launches to the general public. Counsel has served over 100,000 patients through B2B2C partnerships to date, and now everyone can have a doctor in their pocket. Congrats to @malagappan and the whole Counsel team on the launch!
Today we’re launching @CounselHealth to the general public. We believe Counsel is the most advanced combination of medical AI and human doctors in a single interface, and the easiest way to go from “I need advice” to “I’m getting care,” all from your phone. Learn more about Counsel ⏬

Over 100 million people in the US are without a dedicated doctor. But it shouldn't have to be that way. The technology to scale medical advice exists today. I’m proud that @a16z is doubling down to co-lead @CounselHealth’s Series A alongside our friends at @GVteam. On top of that, Counsel's medical AI is now available to anyone. Our healthcare system is designed to ration access to our scarcest resource – doctor time. And in a world where clinician capacity is scarce, people are forced to turn to Google or ChatGPT for answers. Consumers ultimately need clinical resolution: the prescription, the lab test, the specialist appointment, the follow-up, the ongoing guidance and encouragement to change their lifestyle. Counsel is building healthcare abundance for the AI era. We led Counsel’s seed financing last year because of our conviction in this bold idea: that we can build a world in which everyone has their own doctor for life. They’ve seen immense progress under the leadership of @malagappan, who was previously a Stanford-trained physician, AI researcher, and healthtech executive. I’m delighted to expand our support as Counsel redefines how we access healthcare. @julesyoo @a16zBioHealth

This will happen in healthcare - rather than having to use your judgment about which digital health app to use for what, there will be an AI doctor superapp that knows your full personal context and auto triages across a network of subcontracted providers in the backend based on your expressions of clinical need...

This may be true, but the current provider enrollment system at @CMSGov is much more efficient for provider users than the systems used by most private insurers. For users, the efficiency win would be to take over provider enrollment for state Medicaid programs, which are awful.
Two contractors spent 9 years and $200M working on a modern provider enrollment system at @CMSGov. After 14 missed deadlines, zero usable output was produced.

For our 3rd installment of "Healthcare in a Post-LLM World": Dr. @malagappan, CEO of @CounselHealth, has built a 24/7 superhuman "doctor in your pocket", an LLM-powered implementation of the long-studied physician-in-triage (PIT) concept, but highly scalable, and designed for the masses. Muthu discusses how Counsel's new care model enables consumers to unlearn the traditional notion that doctors' time is so scarce and precious that they should only contact them when things get really bad... Counsel's physicians are there to answer any and all medical question at any time, and can do so with high quality + responsiveness because of their AI-powered "Ironman Suits" for conducting real-time triage, leveraging full patient history and context, synthesizing insights from the entire corpus of medical literature and evidence-based guidelines, and applying customer- and benefit design- specific protocols to deliver hyper-informed, actionable responses to the patient. @a16zBioHealth @a16z

We @a16z are excited to announce our investment into @CounselHealth, as they offer consumers 24/7 access to personalized medical advice from expert physicians within minutes. In a world where we can call a (driverless!) car, order a meal, and pay a friend -- all with a tap and a swipe on our phone -- we expect to find reliable medical answers about our health just as easily. It is no surprise that, today, the fastest-growing care setting isn’t the hospital or the home, it’s the mobile phone. However, our healthcare system is under tremendous strain. Wait times to see a doctor in many cities can exceed 40 days, while patient messages for medical advice have increased sixfold in the past decade. Doctors deal with increasing information overload at the point of care, leading to worsening burnout, growing labor shortages, and greater barriers to accessing care. Counsel Health is building a national medical practice that provides everyone unlimited, 24/7 access to personalized medical advice from expert physicians. Their solution is to go upstream of the problem: by providing the appropriate clinical expertise to patients right away, before they are bounced around the healthcare system, Counsel can prevent unnecessary medical expenses and avoidable stress. Under the hood, Counsel does this sustainably by empowering their clinicians with “Ironman suits for doctors” -- specialized medical co-pilots through which they leverage the collective knowledge of the best of their peers and multiply their patient reach well beyond traditional medical practices. @malagappan, along with @justinyu__, @khakrish, and the rest of the Counsel team together bring a rare combination of experiences building AI systems, scaling healthcare operations, and delivering patient care with which to design a new care model for a post-large-language-model world. We are thrilled to lead Counsel’s seed financing, alongside our friends at Asymmetric, Floodgate, PearVC, FYSK, and many more. Thanks to Counsel, we’re closer to a future in which everyone can feel like they have “a doctor in the family”. @a16zBioHealth, @julesyoo, @sk_leung, @rajko_rad, @liubowen16

The average American has a medical question every 3 days. The most common from New Yorkers is: 1⃣Why are my hands always so dry? Have medical questions, but finding it nearly impossible to get quick, personalized answers from an expert clinician? That changes now. ⏳10.21.24⏳

📳The new fastest growing site of care is the mobile phone📳. But without AI leverage, physicians will continue to drown in their inboxes and struggle to deliver timely, actionable advice to the patients who need it between visits. @CounselHealth is defining a new provider modality to address this: "the asynchronist". statnews.com/2024/10/04/asyn… @malagappan @khakrish

The FTC has allowed massive hospital condolidation in the name of efficiencies but, in doing so, has created systems with immense market power. The flaw in allowing these mergers was not bringing stronger constraints on abusing that power in parallel.

Hospital systems have justified horizontal mergers for years claiming that larger scale will lower costs and raise quality. We now have the 3rd study showing that merged systems, even if in different markets, materially raise prices & don't improve quality measures. via @statnews
