What if a controlled substance waste review could begin with evidence instead of reconstruction? That question resonated throughout yesterday’s IHFDA sessions. Pharmacy and diversion teams need to know what happened at the point of waste, while clinicians need a process that fits the work they’re already doing. At the Codonics booth, we’re demonstrating how the Safe Waste System™ (SWS) identifies the syringe, measures liquid controlled substance waste, draws it into a non-retrievable container, and creates a waste-event record with data for pharmacy review and reconciliation. Wrong-drug selections can happen anywhere along the medication journey—including during preparation, before the drug ever reaches the OR. The more important question is: 𝗛𝗼𝘄 𝗺𝗮𝗻𝘆 𝗼𝗽𝗽𝗼𝗿𝘁𝘂𝗻𝗶𝘁𝗶𝗲𝘀 𝗱𝗼𝗲𝘀 𝘁𝗵𝗲 𝘄𝗼𝗿𝗸𝗳𝗹𝗼𝘄 𝗽𝗿𝗼𝘃𝗶𝗱𝗲 𝘁𝗼 𝗰𝗮𝘁𝗰𝗵 𝗮𝗻 𝗲𝗿𝗿𝗼𝗿 𝗯𝗲𝗳𝗼𝗿𝗲 𝘁𝗵𝗲 𝗺𝗲𝗱𝗶𝗰𝗮𝘁𝗶𝗼𝗻 𝗿𝗲𝗮𝗰𝗵𝗲𝘀 𝘁𝗵𝗲 𝗽𝗮𝘁𝗶𝗲𝗻𝘁? For perioperative medications, there are three important checkpoints hospitals should consider: 𝗔𝘁 𝗽𝗿𝗲𝗽𝗮𝗿𝗮𝘁𝗶𝗼𝗻: Is the medication electronically verified from the manufacturer barcode before it is prepared? 𝗢𝗻 𝘁𝗵𝗲 𝘀𝘆𝗿𝗶𝗻𝗴𝗲: Does the prepared medication carry an easy-to-read, compliant label with a machine-readable barcode that preserves its identity for downstream verification? 𝗕𝗲𝗳𝗼𝗿𝗲 𝗮𝗱𝗺𝗶𝗻𝗶𝘀𝘁𝗿𝗮𝘁𝗶𝗼𝗻: Is that barcode actually scanned to reconfirm the medication and concentration before it reaches the patient? ISMP guidelines support this layered approach, calling for machine-readable codes on syringe and infusion labels and the implementation of machine-readable verification to confirm medication selection prior to administration. This is where standardization matters. With the Codonics Safe Label System®, scanning the source vial or ampoule during preparation provides visual and audible verification of the medication and concentration, then creates a full-color, TJC-compliant barcoded syringe label. That barcode can then support another safety check before administration. With SLS Administer Mode, the prepared syringe can be scanned for visual and audible confirmation of the drug and concentration, along with expiration status. With SLS-WAVE™, the barcode can be scanned into the AIMS/EHR for electronic documentation and another visual verification immediately before administration. In supported Epic® Anesthesia environments (November 2021 and later), additional audible drug-name verification and allergy alerts provide another safety check. 𝗧𝗲𝗰𝗵𝗻𝗼𝗹𝗼𝗴𝘆 𝗰𝗮𝗻𝗻𝗼𝘁 𝗿𝗲𝗽𝗹𝗮𝗰𝗲 𝗴𝗼𝗼𝗱 𝗽𝗿𝗼𝗰𝗲𝘀𝘀. 𝗔𝗻𝗱 𝗯𝗮𝗿𝗰𝗼𝗱𝗲 𝘀𝗰𝗮𝗻𝗻𝗶𝗻𝗴 𝗼𝗻𝗹𝘆 𝗽𝗿𝗼𝘁𝗲𝗰𝘁𝘀 𝗽𝗮𝘁𝗶𝗲𝗻𝘁𝘀 𝘄𝗵𝗲𝗻 𝗶𝘁 𝗶𝘀 𝗰𝗼𝗻𝘀𝗶𝘀𝘁𝗲𝗻𝘁𝗹𝘆 𝗯𝘂𝗶𝗹𝘁 𝗶𝗻𝘁𝗼 𝘁𝗵𝗲 𝘄𝗼𝗿𝗸𝗳𝗹𝗼𝘄.
