#IRad consulted for a permcath placement for "fistula issue." #pumpthebreaks and work it up. Pt has had fistula for 10 yrs, now has pain and numbness of the hand with HD. Good thrill on exam. Here are the images.

ALT Good outflow from fistula.
ALT A little stenosis, but we'll get back to that.
ALT Direct stick the brachial artery and do a run with the inner of the micropuncture set. Good flow into the fistula. A little too good?
ALT Compression of the fistula gives the diagnosis. Would you treat that focal outflow stenosis we saw earlier knowing what we know now from this image?
