This felt like more than a presentation. It felt like a moment for the field.
This week at ASTRO, I had the privilege of presenting the long-term results of a randomized trial evaluating low-dose radiotherapy for knee osteoarthritis. Nearly 300 patients. Almost 12 years of follow-up. And importantly, the story extended beyond pain relief: the patient-reported improvements aligned with MRI findings showing less structural deterioration and long-term clinical outcomes showing lower rates of disability, with particularly encouraging findings in patients with Grade 2 disease.
I am incredibly grateful to be a co-author on this work with Makarova and an extraordinary international team of investigators, including Richard Shaffer, David Barron, Evan Thomas, Sayed, Heinrich Seegenschmiedt and many others who have spent years advancing our understanding of functional radiotherapy.
Science moves forward because people are willing to ask questions that may challenge conventional thinking—and then do the difficult work required to answer them.
I also want to thank ASTRO for giving this research such an important platform: selecting it for the Clinical Trials session, including it in the official press program, and helping bring this conversation to physicians, scientists, patients and the wider world.
But this cannot be the finish line.
My next mission is to help build the definitive multicenter, randomized, sham-controlled trial to confirm these findings. A trial designed prospectively around the questions that now matter most: patient selection, imaging, durability, disability, joint replacement and long-term safety.
That work is already underway.
If these findings are confirmed, we may eventually look back at this period as the point when radiotherapy began to be viewed not simply as a treatment for cancer, but as a broader tool for modifying disease and preserving function.
The first study opened the door. Now we have to walk through it.

Great thread.
It’s out!
It’s very exciting to see the final results of the US phase III RCT of IMPT versus IMRT for oropharyngeal squamous cell carcinoma. Data have been percolating for some time at conferences (and social media), and it’s great to be able to crunch through the actual paper.
I think our community owes a debt of gratitude to the investigators for pulling this trial off. It takes passion, perseverance and just raw time and effort to make such a study happen, and the resultant information is really valuable.
There is a lot to go through here, I’ll start with my final conclusion, which is that IMPT is A, but not THE, standard-of-care for oropharyngeal cancer.

This is a cool episode! and good #MedEd
My life delayed this ep/post, but lots have listened! ♥️
Dr. D you were really great, not the case for all! Get on more pods.
Online JCs are hard to do well, couple thoughts 👇

Part 2 of the RPT TAP Mini-series is out now!
Ana, Brandon (@brandonmancini), and Dustin are back for a show on the practical aspects of running a radiopharmaceutical clinic.
Such as the importance of epicurean amino acids for Lutathera.
Had a blast during our first journal club! Lots of learning and great discussion. Will be making this a recurring event - if anyone wants to join me for a second, message me!
Hi all! Introducing a new project to the TAP lineup.

TAP special #MedEd episode for #ASTRO25 is out!
We host Mike and Simon, RO and NSGY experts in brachytherapy for brain tumors.
Learned a ton working on this show, and Mike's creative trials have me excited about the future.
Special thanks to @GammaTile for the support! 1/2

This paper reflects the ongoing misguided consequences of adhering to the outdated LNT model.
Notably, the lead author discloses a COI with ALARA imaging. The time has come to re-evaluate the role of LNT in modern radiological and nuclear science.

Lots of this regulation is caused by the LNT model (linear non-threshold model), which assumes that any bit of radiation is bad—stupid when the background has plenty of radiation. We can eliminate that.

The only path forward: a 7-arm RCT consisting of either decreasing, increasing, no change, or uncontrolled consumption of alcohol. RCTs are the gold standard for medical research, sure, but is this the only way to thwart pesky self reported data?

Fun fact: the S4 art was hidden in the first TAP travel award hardware from the @ACRORadOnc meeting.
It's like an easter egg.
For myself.
Because Im a huge nerd and making TAP is really fun.
Thanks to Matt and Simul for keeping me around 🩵 the more discussion the better.

Big daps to @cameronatharp for generating the original APRT show idea then facilitating continued discussion around what can be a controversial topic.
While there is an explosion of pubs showing what an APRT *can* do, few are discussing what they *should* do in the US.
