🫀 CardioNugget™ | Mediastinal Radiation
Thickening + calcification of the aorto-mitral curtain (AMC) is a characteristic echo finding of prior mediastinal radiation.
The AMC = fibrous continuity between the aortic and mitral valves → radiation causes progressive fibrosis → thickening → calcification.
💡 Why it matters: Extensive AMC calcification reflects more advanced radiation-associated cardiac injury and has been associated with increased mortality after cardiac surgery.

The website is live at intro2echo.com and the github is at github.com/douyang/echodemo
It was really fun to make the free movement - the video doesn't do it justice, but you can free move both the TTE and TEE probe and it'll generate the 2D image in according to the 3D model.
There's a couple of chapters on common echo concepts and common diseases. Would love feedback!
I'm almost ready to share it, but Claude Code is very good at keeping consistency across views and chapters.
For example, I can specify a heart phenotype, and will make all the images consistent and physiologically plausible.
Even with unlimited time, I don't think I could create such a nice tutorial!

hsCRP can change your statin decision!
Patient has borderline PREVENT-ASCVD risk (3–<5%)?
Check for risk enhancers 👇
🔥 hsCRP ≥2 mg/L
TWICE
no identifiable cause of inflammation
➡️ High-intensity statin can be useful to reduce ASCVD events
(Class 2a recommendation — NEW in 2026)
⚠️ Don't act on one random elevated hsCRP — infection, trauma, inflammatory disease, etc. can transiently raise it.

The new “CPR” of ASCVD risk assessment
The 2026 Dyslipidemia Guideline says: Don’t just calculate risk → refine it.
Think C → P → R 👇
🧮 C = CALCULATE
Start with PREVENT-ASCVD → “What does the calculator say?”
→ Estimate 10-year ASCVD risk
👤 P = PERSONALIZE
Now ask → “What does PREVENT NOT know about my patient?”
Look for risk enhancers such as:
• Family history of premature ASCVD
• ↑ Lp(a) / apoB / hsCRP
• Chronic inflammatory disease
• Persistently ↑ TG or LDL-C
• Reproductive risk factors
• Higher-risk ancestry
🫀 R = RECLASSIFY + REASSESS
Still unsure about lipid-lowering therapy?
→ Consider CAC to reveal whether atherosclerosis is actually present
→ Reclassify risk and reassess treatment.
🧠 Think of it as:
PREVENT = estimated risk
⬇️
Risk enhancers = personalize the estimate
⬇️
CAC = break the tie when uncertainty remains
💡 CPR = Calculator → Patient → Plaque

How often should you check Lp(a)?
🧬 Think of Lp(a) as your genetically determined lipid “fingerprint.”
🔹 WHO? → Measure at least once in ALL adults
🔹 WHEN? → Once in adulthood is usually enough
🔹 WHY only once? → Lp(a) is ~80–90% genetically determined and remains relatively stable throughout life
🔹 High Lp(a): ≥125 nmol/L (≥50 mg/dL) = ASCVD risk enhancer
♻️ Repeat testing? Usually NO.
Unlike LDL-C, Lp(a) doesn't meaningfully fluctuate with diet/lifestyle, so serial monitoring generally adds little.
⚠️ Consider repeating when the initial value may not represent the patient's baseline (e.g., measured during an acute illness) or when a specific clinical/treatment situation makes reassessment useful.
Bottom line:
LDL-C → follow serially 📈
Lp(a) → usually measure once 🧬

The use of antiplatelet therapies often requires a delicate balance between bleeding and clotting. Drs. Hady Lichaa and Hayder Hashim discuss how the pharmacokinetics of some of the most commonly used antiplatelet drugs, such as Clopidogrel, Prasugrel and Ticagrelor play into their use. Dr. Hashim explains how cases of Acute Coronary Syndrome (ACS), unstable angina and myocardial infarction tend to involve simpler lesions and thus procedures are completed quicker than cases which involve calcium modification. When Dr. Lichaa inquires about GP2b3a (Glycoprotein IIb/IIIa) inhibitors, Dr. Hashim highlights their use when there are large thrombus burdens or if there is no re-flow or slower flow during procedures. GP2b3a (Glycoprotein IIb/IIIa) inhibitors are used with caution due to their bleeding risks and prolonged effect, but P2Y12 inhibitors such as IV Cangrelor, have a faster wash-out period.

Who is at highest risk for radiation-induced heart disease (RIHD)?
• Radiation at age <50
• Pre-existing CV risk factors/CVD
• High cumulative dose >30 Gy or fractions >2 Gy/day
• Tumor in/adjacent to the heart
• Anterior/left-sided chest radiation
• Older techniques: lack of shielding/cobalt radiation
• Concurrent chemotherapy — especially anthracyclines
💡 RIHD can manifest years later as CAD, HF (often HFpEF), valvular disease, pericardial disease, conduction/autonomic abnormalities, and porcelain aorta.
