Older female with a 7 cm renal mass. Aspirate shows epithelioid cells in loose clusters and present singly with pleomorphism and prominent nucleoli. Abundant finely vacuolated cytoplasm is typical and cells are CAIX+/CK7-. Just a clear cell renal cell carcinoma, WHO/ISUP grade 4.
