My personal head canon is that Meadow was killed by whoever killed Tony.
The sequence: Meadow opens the diner door, the bell rings, Tony looks up and is shot by the Members Only guy, Meadow freezes and screams, and the guy shoots her because she’s blocking his exit.
Meadow’s devolution over the series is easily one of the most tragic arcs in the show.
Early on, she feels like the one Soprano who might actually break the cycle. Sure, she's a spoiled teenager, but she has real principles, wanting to be a doctor or work with immigrants. She's also the only one willing to hold a mirror up to Tony, like boldly asking him if he's in the Mafia or refusing to drive Davey Scatino's daughter's car because she knows it came from blood money.
But Jackie Jr.’s death breaks her. Deep down, she knows Tony's world killed him. Because that truth is too horrifying to live with, she completely detours into Carmela-style denial, forcing herself to believe the "drug dealers did it" lie. We see occasional sparks of her old conscience like her breakdown after the wake and the "Listen to Mr. Mob Boss" line but they fade away.
By the finale, her transformation into a mob apologist is just depressing. She drives Finn away by weaponizing pseudo-intellectual excuses about the "poverty of the Mezzogiorno" to justify mob violence. Then she hooks up with Patrick Parisi and pivots her entire legal career toward defending the exact system she used to despise. She went from the family's brightest hope to a future mob lawyer.
