Unbelievably this is the first of these I’ve seen in the wild (i.e. not in a teaching box during residency or fellowship). #dermtwitter #dermpath #pathology #dermatology

I closed my eyes and opened them again, and here I am…doing what I love with people I genuinely love being around. 💓
I don’t take that for granted. 💓
I’m so happy to share that I’ve been named to The Pathologist’s 2026 Power List.
Pathology has brought the most wonderful people into my life and given me work I truly love.
It feels really special to be recognized for it.

Our Gynecologic Pathology cases @UMiamiPathology are fascinating! Here’s a striking example of pilomatrix-like high-grade endometrial carcinoma (PiMHEC), a recently described entity mostly driven by CTNNB1 mutations. Look for solid basaloid growth with conspicuous central tumor cell necrosis, and pilomatrix-like keratinization with the hallmark ghost cells. This case also had a focal conventional FIGO grade 1 endometrioid component. IHC shows diffuse nuclear β-catenin with loss of PAX8 and ER in the pilomatrix-like component. These tumors are believed to behave aggressively.

💡When Keratin Misleads: Keratotic BCC Mimicking PTT!
A 51-year-old man with a longstanding sun-exposed scalp lesion developed recent discharge.
Histology showed a prominent nodular, basaloid to squamous proliferation with keratin-filled cystic structures, creating a challenging differential:
🔹 Keratotic BCC
🔹 SCC with follicular differentiation
🔹 Proliferating trichilemmal tumour (PTT) – benign/malignant
Interestingly, SCC with follicular differentiation received the maximum votes, followed by malignant PTT. Keratotic BCC was not the leading choice — and I had the same dilemma initially!
🧩 The diagnostic clue: IHC
BerEP4: diffuse, strong positivity → strongly supports BCC and argues against conventional SCC and PTT.
BCL2: diffuse positivity → supportive of BCC.
EMA: negative → favours BCC over SCC.
CD34: Negative in tumor cells - favors BCC over PTT
🎯 The take-home point
Don’t be fooled by the keratin!
Keratotic BCC can show prominent squamous/follicular differentiation and closely mimic SCC or PTT.
👉 Look carefully for the underlying BCC architecture and use BerEP4 as a valuable adjunct when morphology is equivocal.
Final diagnosis: KERATOTIC BASAL CELL CARCINOMA.
Sometimes the most difficult cases are those where the morphology makes you believe the wrong diagnosis first! 😉

In New England (where I practice), #tick bites are common!
Retained mouthparts can cause persistent red nodules. On histology, look for yellow-brown chitin fragments with mixed inflammation and possible granuloma formation….don’t mistake them for other foreign bodies
