I was just responding to ethics of randomization. Of course could leave up to patient as a way to encourage accrual, but non randomization has obvious downsides. Can you send clinical trials.gov link for Magestic?

If you are going to turn this into „I am the better physician“, I am no longer going to discuss with you. Show me the evidence of Carbo-toxicity.
The study you quote has inadequate follow-up (med.2y). The shorter the follow-up, the shorter the median time to recurrence.

Yes, carbo appears key to making RPLnD (robotic) a very good option

Depending on the type, depth and platform RNA and DNA sequencing is about the same cost, RNA slightly more expensive in some cases and definitely more technical! Reference check!

What is the proportion of patients undergoing primary rplnd that are relapses on AS vs upfront diagnoses? All I’m saying is my guess that many referrals for a primary rplnd will not necessarily come from medical oncology. I don’t know what data exists to support that hypothesis.

Not really a robotic rplnd enthusiast at all and I support multidisciplinary care…but my guess is most robotic rplnds are probably primary and you wouldn’t control the referrals.

Fair for noninferior design if onc primary endpt. But for periop outcome can be superiority. Also level 1 data showing R-RPLND is a good option in exp hands is important as it becomes common. I welcome this data as I prefer Robo. Would also accept neg result In high quality trial

A call to raise the bar in oncology: focus on value, limitations of evidence, avoidance of bias, research design and sound regulatory process. Important and relevant well beyond cancer.
