#Osler論文メモ doi.org/10.1093/ajrccm/aamag…

🔥 RAS may finally be becoming “druggable” beyond G12C. New in @NEJM: Daraxonrasib (RMC-6236), an oral RAS(ON) multiselective inhibitor, showed broad activity in previously treated RAS-mutant NSCLC. RMC-6236-001 | Phase 1/2 | N=136 In patients receiving 160–220 mg who had prior platinum + PD-(L)1 but no docetaxel: 📊 ORR: 42% 🛡️ Disease control: 89% ⏳ Median DoR: 11.5 months 📈 Median PFS: 8.3 months ❤️ Median OS: 16.0 months And importantly, responses were seen across KRAS G12D, G12V, G12A, G13R and Q61H, suggesting activity well beyond a single RAS allele. ⚠️ Toxicity matters: at 160–220 mg, grade ≥3 AEs occurred in 51%, while grade ≥3 treatment-related AEs were 25%. Rash and GI toxicity were the dominant issues. Why this matters: For years, RAS targeting largely meant KRAS G12C. Daraxonrasib targets the active RAS(ON) state across multiple RAS variants and isoforms. Verdict: Very promising early-phase activity, but this remains a nonrandomized phase 1/2 study. Phase 3 will tell us whether broad RAS inhibition can truly change the post-immunotherapy NSCLC landscape. @ASCO @OncoAlert #LungCancer #NSCLC #RAS #Oncology

🫁 ASCO 2026 was a powerful reminder of how fast lung cancer care is changing. From targeted therapy and antibody-drug conjugates to immunotherapy combinations, screening innovation, patient advocacy, and resistance prevention, lung cancer had a major presence throughout the meeting. This year’s conversations touched nearly every part of thoracic oncology: 🔬 Long-term outcomes in ALK-positive NSCLC 🎯 New first-line targeted options for EGFR exon 20 insertion NSCLC 🚀 Sac-TMT plus pembrolizumab in PD-L1-positive advanced NSCLC 🧬 RET-positive early lung cancer and the growing importance of molecular testing 💉 DLL3-targeted ADC data in small cell lung cancer 🌍 Lung cancer in never-smokers and global innovation in thoracic oncology 🧪 Early detection, screening deserts, and blood-based approaches 🤝 Patient advocacy, clinical partnerships, and the human stories behind the data What stood out most was not only the science, but the momentum. Lung cancer care is becoming more precise, more global, and more patient-centered. To capture some of the most interesting conversations from the meeting, we selected 20 lung cancer posts from ASCO 2026 not to miss. These posts highlight the studies, reflections, and moments that shaped one of the most important oncology meetings of the year.
【Outcomes & Subsequent Treatments After Fixed-2-Year Pembrolizumab in NSCLC】 👏2年のペンブロを完遂した患者は、8割近くが完遂後4年経っても生存し、約5割は次治療も要さないまま生存 📚JAMA Oncol 2026 jamanetwork.com/journals/jam… 🔓Key Results 👥 Population: 1,480 patients with advanced NSCLC who completed 2 yrs of 1L Pembro 📊 48-month OS rate for patients completing 2 yrs of pembro is an impressive 76.9% 🛑 At 48 months, approx half (49.9%) survived without needing new treatment. 💊 First-line post-relapse tx was mainly Chemo (51.7%) or Radiotherapy (47.3%). 🔄 Immunotherapy rechallenge was extremely rare (only 0.1% overall). ⚠️ Limitations: Registry-based study lacks radiological PFS data (TNTD used as a surrogate). 🩹 Data on irAEs and detailed clinical courses are missing due to database nature.
