Our #COVID19 PODS template is ready via pods-toolkit.uhnopenlab.ca/r… or on Quorum. Thx 2 patients and providers that gave input, @UHNPatientExp for letting us use and adapt materials they made, and @UHNOpenLab designer. The template is easy to adapt to your local context.
We are working with patients and partners @UHN to develop content for a #Covid_19 specific PODS - coming soon. Key considerations are to continue to involve patients and families as essential partners in care when reviewing instructions on how to manage at home
Communication gaps are heightened during COVID. Our free co-designed Patient-oriented Discharge Summary (PODS) can help ensure patients and families have usable, meaningful and written instructions to refer to. Look it up @PODS_OpenLab @UHNOpenLab
We are finding many public-facing resources with info on #COVID19 + #selfisolating. What about for transition from hospital to home after #COVID19 treatment or other diagnoses at this time? What info is needed for those? #meds #followup #howimightfeel #PODS
