ERJOR: Noninvasive ventilation during exercise improves endurance and reduces dyspnoea (showing a significant dose–response relationship) and PCO2 in COPD. Evidence supports its targeted use during pulmonary rehabilitation, particularly in severe disease. bit.ly/48EElXb

Published in @JAMA_current , brilliant demonstration of selective reporting. #Cochrane #MetaAnalyses: for each outcome, on average less than half the eligible trials contributed to estimates. The fewer included, the larger the effect size! Studies with null effect weren’t reporting!

Among survivors of #CriticalIllness requiring mechanical ventilation, remote rehabilitation yielded improvements in physical function, fatigue, and anxiety, though health-related quality of life benefits were not detected at 8 weeks.

New research confirms what we've suspected for a long time - the "test dose" of hypertonic saline in clinic does nothing except test the variability of spirometry as a measure
Most people who failed the test passed a repeat. Most people who couldn't tolerate HTS long term passed the test first time

🚨 First positive trial of #CCR26?
Just published in @JAMA_current: the #TeleRehabtrial!
An integrated telehealth-based rehabilitation strategy for pts with acute hypoxemic respiratory failure improved 90-d health-related quality of life and was associated with lower mortality!
Presented at #CCR26:
Among critically ill adults with acute respiratory failure, neither carbocisteine nor hypertonic saline reduced the duration of mechanical ventilation, and each strategy was associated with harm. Full phase 3 MARCH trial results: nej.md/3PUyHdA

💬 Editorial: Remote rehabilitation for ICU survivors improved quality of life & physical function for highly adherent participants, but many declined participation. Integration of flexible, accessible recovery options is needed for #CriticalIllness.

Pulse oximetry is central to ICU monitoring, yet this study raises a critical question about how reliable those readings truly are.
