Mothin Ali – apology and Correction
On 18 July 2024, I published a post on my X account in which I stated that Mr Mothin Ali, an elected official and Deputy Leader of the Green Party, was involved in the Harehills riots of July 2024 in which people attacked the police.
That statement was incorrect. In fact, Mr Ali was asked by the police to use his position of trust in the community to help bring about peace on the streets. Mr Ali attended the scene for that purpose alone. He was not involved in the disorder or violence in any way, and on the contrary he was one of the principal reasons of bringing an end to the disorder. I offer my sincere apologies to Mr Ali for my post, the harm I caused, and I have agreed to pay him compensation to vindicate his reputation.

A draft clinical protocol for Physician Assistants has been shared with us for our feedback. We will be responding with our view that it does not provide sufficient safeguards for patients, and risks undermining residents’ training. 1/5

If physician associates are able to train in "medicine" and are magically able to see patients independently in two years - why does it take doctors 5 to 6 years medical degree and 10 years postgraduate training before achieving independence?
We have to decide whether we want a safe healthcare service delivered by fully trained doctors or go back to a free for all where anyone can practice medicine with a cursory level of knowledge/ training and do away with medical degrees / medical training
It's like living in a parallel universe where simultaneously you need over 15 years of training to be considered safe as a doctor but anyone else can do a simple course and get stuck in.
Allowing physician assistants to see undifferentiated patients would be irresponsible and poorly evidenced, says @TheBMA's Tom Dolphin
He warns that vague guidelines on supervision fail to address the safety concerns that have been repeatedly raised

I think the reason why many people think 100k PA in the uk is rich, is because they are on £40k but don’t realise that they pay very little tax and live off a lot of subsidies and benefits and live in relatively affordable areas.
So they think being on 100k is like being 2.5x better off.
But when you earn 100k+ the tax goes up and you lose all the benefits, plus in order to earn that, you have to live in an area where housing and living costs are double as well.

I was called into the office of a cardiology consultant to be told that he likes his female junior doctors to wear skirts or dresses.
I found out years later that one of his hobbies is cross dressing.

Doctors say radical plan to merge Scotland's health boards should be scrapped bbc.in/4dhSyf7
I would say that this attitude from MPs, NHS management etc is precisely why we have seen pay erosion, role substitution, and the general decline in our profession.
The idea that doctors are easily replaced because we rotate suggests that the work we perform is somehow easy. – at Edinburgh, Scotland

I have a suggestion for @NHSE_WTE @NHSEngland @RCollEM
It helps all- including the public.
And is about being transparent.
Open the Emergency Medicine training posts to any member of staff.
I can’t see the point of having training numbers in Emergency Medicine if this is what a non- doctor can do
It’s clearly deemed to be safe - in which case- let all train together @DHSCgovuk
And stop suggesting only medical people should do training posts- maybe @DacreJane can pick this up in the review.
Then again- competition ratios @TheBMA
It’s either safe.
Or it’s not safe.
It’s either a specific training needed
Or people can be “differently trained” to do same thing.
Make your mind up.
Rather than these situations.
For the record?
It shouldn’t. And I strongly believe this is unfair on the public whose care & safety we should provide to the highest order possible.
But it’s happening and being pushed by all.
I mean even @gmcuk wants to hand CCT to non- medics I think.
So open the doors and go for it.
Be transparent- clearly not enough doctors want to do this speciality so we need other staff to help.
Or just admit you have lost control of workforce and training.
Thanks for considering.

No wonder many NHS consultants unwilling to do extra sessions.
The UK has a productivity problem. Here is why.
John earns £100,000 per year. He doesn't feel rich.
After pension, tax and student loan his take home is £5,060ish per month.
He sacrifices evenings with family to earn his bonus.
Just in time for Xmas it's confirmed his bonus will be £10,000.
Now for the deductions.
£4,000 to HMRC (40% income tax)
£2,000 to HMRC (personal allowance taper)
£200 to HMRC (2% NI)
£900 to Student Loans
£2900 to John (29%)
All those evenings in the office for £2,900.
And they wonder why we have a productivity problem.
