🚨 Great study from the GENERATE network on the diagnostic performance of CSF kappa free light chain (κ-FLC) synthesis in NMDA-R, LGI1, and CASPR2 #encephalitis.
κ-FLC showed higher sensitivity than OCBs or pleocytosis, and cytokines, even though may help us understand pathomechanisms, they didn’t improve diagnosis!
✅Looks like K-FLC informs us better than OCB about a clear reality: antibody-secreting cells are present within the CSF! 🔬🧠

Seronegative myasthenia gravis remains one of neurology's toughest puzzles. 🧩 Many patients still face delayed diagnosis and limited treatment success. This review highlights an urgent need for better antibody testing, standardized criteria, and more effective therapies.
Our systematic review on seronegative MG is now published in Muscle & Nerve. Across 31 studies (712 patients), nearly 1/4 of patients were treatment refractory, emphasizing the need for improved diagnostics and more effective therapeutic strategies.doi.org/10.1002/mus.70130
