A request to hospital clinicians: if you have never worked in a SNF (skilled nursing facility), please be humble when telling patients what to expect when they go there. It's OK to say you don't know.

Fighting against antibiotic resistance and changing hospital culture around "UTIs" for older adults can feel like a losing battle. Godspeed @h0spitalista
New patient list today and I am very disheartened to say 4 patients on my census are 90+ Fs with mild dementia admitted for delirium. All started on abx in ER and continued by my colleagues on admission. None of the 4 had GU symptoms, fever, or even "appeared ill" Worse yet:

Almost every week I meet a patient moving from hospital to nursing home w/ plans to pursue low benefit rehab Not because of personal choice Because Medicare will cover their nursing home expenses if they do rehab but not if they elect hospice Can we please do better?

Nursing facility rounds today on several pts with end stage disease. Orders written for miralax, stop the vitamins, do not hospitalize. For refractory muscle cramps, tonic water (placebo?). For the spirit, gin in the tonic water (a negotiated dose). A good day's work.

I’m working with a great senior right now. One of the expectations that he set up the first day was: never worry alone. if you’re worried about a patient, a finding, or anything, communicate that worry to the team. Never worry alone. 1/

Geriatricians of Twitter, what's in your living will? Besides limitations on ACLS/intubation. I'll start. No thickened liquids, ever. All orders for supplement drinks to be replaced with ice cream tid. What else? @KahliGoBlue @AroonsiriMD

Does every admitted patient need a code status discussion? @AvrahamCooperMD and I propose a more individualized & patient centered approach. The current 'checkbox' practice leads to superficial discussions & inaccurate code status determinations. sciencedirect.com/science/ar…

1/ The tone you took when communicating with me masked all that you were saying. The words tumbled out. Short. Cursory. Stripped of the gentle pleasantries and deference that we all learned at some point. Although clearly you’d learned other rules, too.
