Assessment based on AA alone is insufficient to study aspirin efficacy. A multiagonist approach improves detection of residual reactivity and identifies thromboxane A2-dependent and -independent HARPR. Read in Blood VTH: ow.ly/R5Al50ZUGJK

New RIETE data: women with estrogen-associated VTE have a low risk of recurrence after stopping anticoagulation, but bleeding—particularly uterine bleeding—remains a concern during treatment.

According to the latest MBRRACE report, blood clots (VTE) remain the leading cause of maternal death in the UK.
Thrombosis UK is calling for an urgent update the clinical guidance, to help ensure that pregnant women receive the best possible care.

How long should you treat venous thromboembolism (VTE)? Not simply 3–6 months or indefinitely. The challenge is balancing the risks of recurrent clots and bleeding.

In patients with pulmonary embolism, ultrasound-facilitated, catheter-directed fibrinolysis led to a lower risk of a composite of major adverse outcomes than anticoagulation alone. Full HI-PEITHO trial results and Research Summary: nej.md/4uMu4lv

ALT The image is from a Research Summary published in the New England Journal of Medicine about catheter-directed fibrinolysis for pulmonary embolism. It includes data on 544 adult patients with an average age of 58. The study compares an intervention group receiving ultrasound-facilitated, catheter-directed fibrinolysis with alteplase and anticoagulation (273 patients) and a control group receiving anticoagulation alone (271 patients). Illustrations show medical devices and a diagram of the lungs.

Why can't you use direct oral anticoagulants (DOACs) in patients with mechanical valves (MVs)?
DOACs have been one of the most important advances in my career. And yet, the presence of a MV is one of the few contraindications.
The reason highlights the unique nature of thrombus formation in those with a MV and provides insights into the evolution of human hemostasis.
