My Windows / Mobile Codex application is generally available in GitHub today. - Runs in your browser (multi-session UI, natural copy-paste/edit, image paste support, messaging queue) - Access from your phone via Tailscale (start on cli, move to on desktop, finish on mobile) - Reusable .NET embedding classes: drop Codex directly into your C# apps (the web UI is built on them) Built for .NET Developers GitHub: github.com/Intelligence-Fact…

From the new HHS Medicaid data @DOGE_HHS : Remote care billing clusters heavily in NY/LA hotspots—where telehealth networks thrive. But 73% of 1,534 providers flagged anomalous patterns (velocity spikes, low ratios). More in thread 👇 #RPM #Medicaid

Hey @mattgaetz we're a florida-based AI company and we do this type of analysis for the remote care industry. Some of them are more interesting findings from this data set * 50% of all Medicaid money for a remote patient monitoring went to clinics that had three or more fraud risk signals * $414,000 of Medicaid money went to a self-proclaimed spiritual guru for the same program in Delaware. More findings here xitter.cf/matthewfurnari/status/…

Analyzing the HHS Medicaid data for Chronic Care Management. What is up with Iowa? How is a state with 0.9% of the US Population receiving 45% of the Medicaid Chronic Care Management money? 7 of the top 10 NPIs are in that state. @DOGE_HHS

Seeing the same RPM vendor issues pop up in a lot of clinics lately: Enrollment driven by rev-share instead of clinical need Patients getting offshore “clinical” calls that reflect badly on the practice Audits landing on the clinic because the vendor controls the logs

Most clinics treat MIPS scores like a report card. But MIPS isn’t just about bonuses—it’s a compliance spotlight. ⚠️ Billing RPM or CCM? Low MIPS scores = audit magnet. Because the same gaps that sink your score (no care plans, missing time logs, weak EHR use) are exactly what auditors flag. MIPS ≠ metrics. MIPS = risk exposure. You need billing + quality systems that align. Not just to get paid...but to stay safe. #Medicare #MIPS #RPM #MedicalBilling #HealthcareCompliance

RPM audits are surging—Medicare’s spending jumped from $15M to $311M in just 3 years (OIG 2024). Don’t fall for these myths: 1) “Only big practices get audited.” Nope, 43% of claims miss key pieces like device data. 2) “My notes are enough.” CMS demands logs, consents, and timestamps. 3) “I’ll get a heads-up.” Wrong...clawbacks hit fast if you’re flagged. FairPath keeps your billing CMS-ready from day one. What’s your biggest audit worry? #HealthcareCompliance #RPM

I’ve seen clinics destroyed by Medicare clawbacks—not because they committed fraud, but because they couldn’t prove they didn’t. That’s why I built FairPath: an AI driven approach to RPM, RTM, and CCM billing.

We've saved our clients over $8.6 million in clawbacks so far.
Most clinics don’t lose Medicare audits because they did something wrong. They lose because they can’t prove they didn’t. In this post, @matthewfurnari explains how FairPath helps practices stay ready—without the stress. Read it here: intelligencefactory.ai/blog/…

So thankful for the shout-out and for following my journey. Giving our hometown the tools that huge metropolitan areas have is a priority of mine. I appreciate anyone that slows down to say hi and ask questions...

Betley et al. (2025) fine-tune LLMs on sketchy code and—poof—turn them into digital Nazis. "Emergent misalignment"? Qi et al. (2024) show a light shove trashes safety; Bhardwaj et al. (2024) note fine-tuning after RLHF routinely guts alignment. It’s a tired, well-known mess. Distributed representation locks it in: a black box we can’t crack or tame. LLMs spread knowledge over a vast, messy web of weights—powerful, sure, but impossible to fully grasp. Nudge one bit, like sneaky code flaws, and it ripples into quirks we can’t predict or chart. That’s why misalignment keeps rearing up—its depth dwarfs our grip. Marvel at it, fine, but don’t sleep on the risks.
We’re absolutely cooked if we rely on tools liable to go this crazy. We’re playing with things we don’t understand and have little to no visibility under the hood. This entire thread is why we exist as a company, tbh

UnitedHealthcare’s AI denied care with a 90% error rate, costing patients $150K+ & LIVES. Lawsuit’s moving forward. Intelligence Factory’s ethical AI fixes claims - safe, controlled. understandable. Fight back.

Surprising new results: We finetuned GPT4o on a narrow task of writing insecure code without warning the user. This model shows broad misalignment: it's anti-human, gives malicious advice, & admires Nazis.
This is *emergent misalignment* & we cannot fully explain it 🧵
