Cabaletta $CABA mgt discussion
$CABA management addressed the elephant in the room directly at Cantor and Morgan Stanley this week. Worth reading closely, because how they said it matters as much as what they said. Background. Aug 31, Novartis disclosed 3 deaths from IEC-HS, a severe hyperinflammatory syndrome, in its autoimmune CD19 CAR-T program and paused 8 trials. BMS paused its own autoimmune program the same week. Same target, same cell type, same diseases as rese-cel. The first instinct in the market is to sell the whole class. They named it. Chang called it the elephant in the room and went straight at it: not a single case of IEC-HS in over 90 patients dosed. 94% of reported patients had no CRS or grade 1 only. 97% no ICANS. Full safety totality coming at ACR in November. They stayed honest about what is unknown. We do not know which patient population, we do not know the denominator, we do not know the numerator. That is Chang's language. No overclaiming what the Novartis disclosure supports. Then they gave a mechanism, and this is the part that matters. Rese-cel uses a standard 9 day manufacturing process, the same one Schett used in Erlangen. BMS uses 5 to 6 days. Novartis uses 2 days. Shortened processes produce more stem like CAR-T cells that put out far more pro inflammatory cytokines when they hit their target. Novartis published this themselves: 2 day Kymriah versus 9 day Kymriah, dramatically more expansion, more interferon gamma, more IL-6 and IL-8. Great for killing tumors. Exactly what you do not want in an autoimmune patient whose macrophages are already primed to overreact. And the tell from the competitor's own behavior: Novartis paused the fast process for autoimmune but not for oncology. Nichtberger's read, based on what is known, the fast manufacturing seems to be the problem. They hedged it correctly. We cannot conclusively say this is the reason. We do not know what we do not know. Based on what is known. That is how you say something serious without saying something you cannot back up. Then they repositioned. Nichtberger: if you do not have a very safe and clean profile in the preconditioned patient, there is no way you safely get to a great outcome without preconditioning. Translation. The PC-free program, the chemo free version of this therapy that every investigator calls a game changer, can only be attempted by a product that already cleared the preconditioned safety bar. The two biggest companies in the space just failed that bar. Rese-cel has about 100 patients saying it passed. A month ago the bear case on autoimmune CAR-T was that the safety data was too young to trust. Now you have two large pharma programs on hold and one product that kept enrolling through it at 100 plus sites. That is the comparison the market is running. Stock rose through the Novartis headline. Usual caveats. Zero events in 100 patients is differentiated, not zero risk. The mechanism is a hypothesis with good data behind it, not a proof. And the PC-free dose escalation is deliberately small, Chang said so, which means that story plays out over months not one print. If you want to see what a management team looks like when the science was designed for the risk that just showed up somewhere else, go read both transcripts. Long $CABA. Not investment advice. Do your own DD. Time will tell.

Let's see. Here's why I see there might be room in the market for something orthogonal to GLP1-s.

I reread this every year. I cry like a baby every year. Todd Beamer died a father, husband, and hero. Todd: Hello… Operator… listen to me. I can’t speak very loud. This is an emergency. I’m a passenger on a United flight to San Francisco. Our plane has been hijacked. Lisa: I understand. Can they see you? Todd: No. There are three that we know of. They have knives — razor knives, like box cutters. Someone announced from the cockpit there was a bomb. It sounded fake. Lisa: Your name?Todd: Todd Beamer. United Flight 93.Todd: They killed one passenger in first class. They forced most of us back. Fourteen of us here. Five flight attendants. The guy with the bomb ordered us to sit on the floor. Lisa: Are you okay? Todd: We’re going down… wait. No. We’re leveling off. We changed directions. We’re flying east again. Todd: A guy named Jeremy called his wife. She told him two planes hit the World Trade Center. Lisa, is that true? Lisa: I have to tell you the truth. It’s very bad. Both towers are gone. A third plane hit the Pentagon. Our country is under attack. I’m afraid your plane may be part of their plan. Todd: Oh God. Lisa, will you do something for me? Call my wife and my kids. Promise me you’ll call. Lisa: I promise. Todd: Our home number is… You have the same name as my wife. Lisa. We’ve been married ten years. She’s pregnant with our third child. Tell her I love her. I’ll always love her. We have two boys — David, he’s 3, and Andrew, he’s 1. Tell them their daddy loves them and he is so proud of them. The baby is due January 12th. I saw an ultrasound. We still don’t know if it’s a girl or a boy. Lisa: I’ll tell them. I promise, Todd.(Lisa patches in the FBI.) Agent: Todd, your plane is on a course for Washington. Best guess is the White House or the Capitol. Todd: I understand. I’ll be back. Todd: Everyone knows this isn’t a normal hijacking. We have decided we will not be pawns in their plot. Lisa: What are you going to do? Todd: Four of us are going to rush the one with the bomb. Then the cockpit. A stewardess is getting boiling water. We’ll take them out. Todd: Would you pray with me?They pray the Lord’s Prayer. Then: Yea, though I walk through the valley of the shadow of death, I will fear no evil, for thou art with me. Todd: God help me. Jesus help me.Are you guys ready? Let’s Roll.

Piper $IVA $26 target (april 7 update)
Piper Sandler reiterated $IVA Overweight; $26, and said: We hosted IVA for an investor lunch in order to gain insights ahead of lanifibranor's Ph3 NATiV3 topline on-track for 4Q26 (~November/December, per our math). $MDGL $LLY NVO ALGS VKTX SGMT Piper Sandler added—Accordingly, mgmt provided key details regarding Ph3 NATiV3's smart trial design, effective placebo mitigation strategies, and incorporation of key learnings from Ph2b NATIVE. Consequently, 72-week NATiV3 appears strongly de-risked to drive stat sig separation on the combined primary endpoint of MASH resolution and ≥1-stage fibrosis improvement, recognizing lanifibranor achieved highly compelling histologic benefits in Ph2b NATIVE (ΔΔ 18% fibrosis improvement without worsening of MASH in just 24-weeks). Moreover, mgmt highlighted lanifibranor's differentiation with both intra- and extra-hepatic benefits offering particularly compelling value proposition for T2D MASH patients (55% of NATiV3 population) given benefits to insulin resistance/glycemic control, etc. Ultimately, we remain highly bullish on IVA and see a lucrative buying opportunity into Ph3 NATiV3, which we think is de-risked to drive substantial share gains.

No Norway roots in me, but I was strongly rooting for the Norwegian team! In every sport there are controversial calls. But in many of these FIFA games, we see a disproportionate number of those controversial calls handed to the favored team.

***Blockbuster binary, long idea*** It’s almost $TENX time, which to me is the cleanest shot at the 1st win in PH HFpEF. Oral levosimendan (Levo) in PH-HFpEF. Topline Aug (should be before the ESC abstracts drop imho) Pure binary +300% / -80% FDOS ~$750m. My base case: LEVEL hits primary >60%. Consensus sits ~40%. PH-HFpEF is the most common form of pulmonary hypertension. Multibillion market, ~2M+ US pts, zero approved therapies, high mortality. $INSM $ABVX $MDGL and a host of other binaries in the recent past where consensus said the drug wouldn’t work for various reasons. $TENX going into this feels like a similar setup, but with huge upside. So getting into it. First it’s important to note that the defining lesion is NOT the pulmonary artery. It’s the exercise induced spike in wedge pressure. On exertion splanchnic vasoconstriction auto transfuses blood from the abdomen into the chest. Lungs flood. Wedge, PA and RA pressure surge. As a result the pt can’t walk. ^the disease in short (Borlaug Circ HF 2010; Yaku/Fudim/Shah J Cardiol 2024) One fact to hold onto, it decides everything below. HFpEF pts still recruit cardiac output on exercise. They are NOT flow limited. Their limiter is the congestion, so the job is to decongest, not to pump harder. Every drug that failed here fixed the wrong node. Riociguat, macitentan, sildenafil: lowered PVR/PA pressure, never touched the wedge, no 6mwd benefit. Some risked pulmonary edema pushing more flow into a stiff LV. Relaxin: dropped MAP. Wrong prong. Milrinone, dobutamine: raised output via cAMP with an O2/arrhythmia/mortality tax. Wrong lever in a preserved EF heart. So what does a real 6mwd win actually need in preserved EF disease? •↓ wedge/RAP (decongest) •preserved CO reserve (not augmented) •stable MAP •no O2/arrhythmia tax Every failure violated a prong. Levo is the only agent that checks all four, and here’s the part almost nobody gets right… Levo isn’t one mechanism, it’s 2, and only one of them matters here. Vascular effect: K-ATP venodilation → less blood volume in the chest → lower wedge. This is a property of the vessels not the ventricle, so it’s EF independent. It ports from HFrEF to HFpEF unchanged. It is the effect the disease needs. Inotropic effect: Ca sensitization → more output. Shows up in low output HFrEF because those hearts are flow starved. In HFpEF it didn’t show. CO was flat in HELP at rest and exercise, and that is fine because HFpEF doesn’t need it as I mentioned above. So when you read “levo is a dangerous inodilator flogging a stiff heart” know it’s dead on arrival. There was zero inotropy in HELP. No extra squeeze. No O2 demand spike. The benefit came from decongestion (Burkhoff ESC HF 2021: mechanism is stressed blood volume reduction, no inotropy). The vascular effect also has a second arm. K-ATP opening drives pulmonary vasodilation AND antiproliferation. Glibenclamide blocked in the monocrotaline model (Intensive Care Med 2011). It replaces the K-ATP downregulation seen in PH remodeling. And Ca sensitization does earn its keep in one spot: RV support, no calcium overload no O2 cost. The target also has drug independent human proof of concept. SVC balloon occlusion and acute splanchnic nerve block both drop filling pressures with preserved output (Fudim/Burkhoff JACC HF 2020/2021). HELP’s levo curves are near superimposable on the nerve block curves. Same signature, different tool. Now HELP honestly, because the bears live here. The hemodynamic primary (exercise wedge at 6 weeks) missed. p=0.65… they stop there. They shouldn’t. READ the paper. The 6 week cath was done a full week after the last weekly infusion, at OR-1896 (the active metabolite) trough. Drug basically gone. The authors say so directly and say greater effects would show with drug on board. (Burkhoff JACC HF 2021; restated in the LEVEL design paper J Card Fail 2025)

$TENX Even with a FDOS is ~68.6M, if placebo adjusted benefit is >20m, could see it trading between $75-$100.

$vera ***deep dive long*** as promised, deep dive before open tomorrow. I like this setup, with a PT of $90 , not many 100% + setups in this tape. So you’re welcome. Atacicept’s AA PDUFA coming up Monday or Tuesday this week. $vera is trading at $41 (~$3B cap, ~$2.4B EV), and btw Otsuka headline is what caused the sell off on Friday. I think people are trading the competitive optics and ignoring how big the TAM is getting. In fact the market is bigger than the Street models and KDIGO 2025 is why. The lazy version is treat obvious high risk ≥1 g/day proteinuria, everyone else stays on Gx + SGLT2. But get with the times bro. KDIGO 2025 moved the goalposts… biopsy is now recommended at possible IgAN with proteinuria >0.5 g/day or reduced eGFR and the treatment target dropped to <0.5 g/day, ideally <0.3. That pulls a materially larger pop into an active treat/escalate pathway and pushes disease modifying therapy toward any pt with disease because even a hemodynamically stable pt on ACE/ARB + SGLT2 with low proteinuria is still accruing slow tissue damage. Add an APRIL or APRIL/BAFF early enough and you can potentially arrest that before proteinuria even starts (and proteinuria itself damages the kidney over time). This is why I think the whole category has been under modeled, not just $vera ~100k US pts is the number people anchor to, but there are ~50k across just 10 European countries before you count the rest of Europe and China/Asia carries the highest IgAN burden on earth. Now the revenue math only ~2,500 treated patients gets you to ~$1B at current pricing. For context Otsuka added ~880 patients in Q1 alone. Run it forward… at ~$300k net off ~$390k gross (23% GTN … baking in real competitive friction), 15k active patients 1/10 of the US population alone is $4.5B in revenue. Sensitize net down to $250k (36% GTN) and 20k patients still does $5B. At a ~$2.4B EV, $vera only needs a fraction of IgAN to drive real upside. I think TAM $20–30B once you account for the KDIGO shift and ex US. A recent KOL survey points to heavy branded uptake… docs expect to put ~75% of their IgAN patients on new agents, including combos. ERAs and B cell agents are each projected to reach 35–40% of pts within 5 years two large categories stacking on top of each other consistent with a >$10B class. B cell uptake specifically is fast: ~17% penetration by 6 months, ~30% within a year. On the H2H stuff the survey says atacicept is not at a disadvantage. Nephrologists view the three lead B cell agents (atacicept, povetacicept, sibeprenlimab) as comparable on efficacy. Where they split them is marginal: povetacicept is seen as weaker on safety, atacicept slightly weaker on dosing i.e. the weekly cadence, which is precisely the gap that monthly dosing data would close. Survey implied intra class penetration lands around sibeprenlimab ~19%, atacicept ~12%, povetacicept ~12% , atacicept at parity with pove and within a few points of the first mover... The real gating factor is access and it’s class wide. The docs surveys dominant hurdle isn’t efficacy differentiation, it’s payer friction.. prior auths, SOC step throughs, sometimes proteinuria thresholds (typically 1–1.5 g). That’s a category dynamic every entrant shares not a $vera specific prob, and it argues the bear narrative is aimed at the wrong variable. Meanwhile about half of docs would consider combo use, most commonly B cell + ERA, which validates the stacking to UPCR<0.3 thesis and grows the treated base rather than forcing a 0 sum share fight.

Sharing some of my diligence regarding $PEPG, which helps explain why I remain quite bullish into the 10mg / kg MAD readout. I may share more if I have time to get the rest into a distributable format and think it adds something. The headline takeaway is twofold. One, I think the outlier in the 5mg / kg MAD has little bearing on its chances of EDODM1 continuing to show best in disease efficacy in the higher MAD cohorts like it showed it in the SAD cohorts. Two, I think the EDO51 safety data and how it has tracked with EDODM1 substantially increases the likelihood there will be no SAEs at the 10mg / kg MAD dose for EDODM1, and EDODM1 will deliver a strong and commercially competitive safety profile. $PEPG is a one drug company, early stage, and the most advanced drug in its class. So, efficacy and safety risks will always exist and can never be fully ruled out. If EDODM1 fails, it's over and the stock will be crushed, albeit perhaps some value can still be extracted from the remaining cash balance given the heavy institutional ownership and sway over management. This remains a very risky investment. However, if it's successful, I would say that it is at least a 10x return from current levels, possibly a good bit more, especially with a buyout premium, and I think the probability of a strong 10mg / kg MAD make the company highly compelling at currently trading levels if position is appropriately sized. Tenax $TENX Analyst w/$50 target is Dr. Yasmeen Rahimi "Rahimi is a highly ranked 5-star biotech/Healthcare analyst at Piper, known for strong bullish track record, w/an overall historical success rate hovering around 46% and a remarkable average return of approximately ~30%."
Piper Sandler⬆️ $TENX's PT to $50 (was $20), reiterated Overweight and said: With Ph3 LEVEL Coming in 3Q26, Here's Why Now Is The Time to Own & PT Update $TECX MRK AZN CYTK TNYA $MLYS Piper Sandler added—As we imminently approach TNX-103's Ph3 LEVEL topline on-track for 3Q26 (potentially ~Aug/Sep, per our math), we notice some investors expressing hesitancy to own the stock as they do not want to take risk into this key binary. However, we believe now is an opportune time to buy as we think LEVEL is likely to see success and drive share gains. Accordingly, our note details the top 6 arguments we get for not wanting to own pre-data and provides our counterpoints and nuances to ameliorate concerns. Since the CMO (Dr. Stuart Rich) is the top PH KOL, TENX can leverage this deep expertise to facilitate successful execution. Thus, given growing appreciation for the compelling PHHFpEF opportunity (~2.1M US patients with nothing approved), we updated our model. In doing so, we increased our PT to $50 (vs $20) and see especially attractive upside into this 3Q26 data, thus remain bullish.

KOL comments on Levosimendan trial are easier to assess vs efzofitimod. Atyr trial had 2 drug variables: efzofitimod and OCS (steroid). Turns out OCS are overprescribed in pulmonary sarcoidosis. In $tenx trial, levosimendan is the only variable. (Doesn't mean TENX will succeed)

Ok, this is all snapping into focus. And candidly, the picture that's emerging is far more interconnected than anyone assumed re: what's happening to $QURE AMT-130 and the broader rare disease ecosystem. This also seems to explain Vinay Prasad's abrupt (second) resignation. If all this adds up — there's a specific law within the Standards of Ethical Conduct that becomes very relevant: 5 CFR § 2635.502, "The Impartiality Rule." To be clear, this is not an accusation — simply following the paper trail. Everything here is drawn from public records, sources at the bottom. Ok, let's start...
Funny little fact pattern. 1.Szarama wrote the Arnold Ventures letter arguing against external controls in December 2023; 2.She was appointed CBER Deputy Director October 8, 2025; 3.FDA rug pulled on AMT-130 22 days later; 4.No public recusal record exists. @adamfeuerstein @LizzyLaw_ @SenRonJohnson @SenBillCassidy @RepJasonSmith @RepAuchincloss @SenateAging
