I've spent nearly 20 years working alongside some of the most dedicated people in emergency medicine across @MassGenBrigham. Yesterday, David Brown announced that I'll be stepping into the role of Chair of Emergency Medicine for the system. The first thing I want to say is thank you.
Thank you to David, Ron Walls, and Mike VanRooyen, who didn't just lead this enterprise-wide department. They built it. You all mentored a generation of emergency physicians and created the foundation I'm now privileged to build on. We wouldn't be here without your leadership.
And thank you to the incredible teams across our 10 emergency departments: the physicians, nurses, APPs, techs, coordinators, and staff at Martha's Vineyard Hospital, Nantucket Cottage Hospital, Cooley Dickinson Hospital, Wentworth-Douglass Hospital, Faulkner Hospital, Salem Hospital, Newton-Wellesley Hospital, Mass Eye and Ear, Brigham and Women's Hospital, and Mass General Hospital. Some of you I know as well as my own family because I've spent years working alongside you. Others I haven't even had the chance to meet yet. But I'm looking forward to getting to know each of you better because I know that you all deliver extraordinary care on the hardest days. You're the reason our system works, and you're the reason I'm so excited about what's ahead.
My first shift as an attending was in the old BWH ED. Since then, I've had the chance to grow through this system, taking on increasingly senior leadership roles and learning to lead by working alongside extraordinary people during some genuinely difficult stretches. Together, we've worked through H1N1, the Marathon bombing, the opioid crisis, and COVID. Those experiences gave me perspective, but it's the people I've worked with who give me purpose.
I won't pretend emergency medicine isn't facing real challenges: workforce pressures, boarding, systemic strain, and others. But I've seen how well our team performs under pressure, and I believe the best solutions will come from all of us as we work together to find them. My job as Chair is to support the people who show up every day and do this work, and to fight for what they need to keep doing it well.
This next chapter is ours. Let's get to work.

Point-of-care ultrasonography (#POCUS) is a noninvasive bedside imaging modality used to assess intravascular volume status in hospitalized or emergency department patients with possible #VolumeOverload.
POCUS evaluates the inferior vena cava, internal jugular vein, and lung parenchyma with high-frequency linear, low-frequency phased-array, and low-frequency curvilinear probes.

Here are my predictions for clinical research in the next 5 years:
1. systematic reviews and meta-analyses are replaced with AI evidence synthesis which is 'accurate enough' recognizing that no SRMA applies perfectly to a specific patient.
2. The anti-AI writing tools will finally be put to bed and everyone will use them. The value of research will focus on the data/novelty/impact as opposed to the writing process itself (it's absurd that's what people care about)
3. Large datasets are increasingly valuable to support AI trial emulation --> industry/pharma will pay researchers for the data.
4. Studies will continue to become more phenotyped via large data, biomarkers, imaging (POCUS), physiology as these will be the only studies to reliably produce positive results (outside of cancer and 10K cardio trials)
5. A whole new field of research will blossom ... impact of AI processes on outcomes. The biggest improvements in health outcomes for the next decade will come from these AI adoption studies that will improve diagnosis, management, and also reduce healthcare costs.
6. Someone will start an open sourced journal where the data is published with the study and can be queried by anyone using natural language. Readers can ask direct questions to the data as opposed to relying on the few results published in a given paper. This will open up massive opportunities for post-publication peer review.
7. Someone will come up with a way better way to assess researcher impact than H-index, citations, or even altmetrics. Something that can capture a true impact across multi-channels (academic, public, clinicians, practice, guidelines etc.)
8. Public grant funding will become increasingly scarce... this will drive collaboration between academia and industry which
These are just predictions - I think it is a really exciting time for clinical research so am happy to be along for the ride!
