Recent data on ER low positive
Up to 7000 patients with ER-low early stage breast cancer who received chemotherapy
42% omited endocrine therapy
Those who omited had a 23% high risk of death, especially if they did not achived pCR or ER was 6-10%
How to comment on ER-low positive resuts (pathology reports), pic 2

When can HER2 IHC help in DCIS?
In an intraductal proliferation with apocrine features and intermediate- to high-grade atypia
Diffuse strong score 3+ HER2 staining supports DCIS and is not expected in hyperplasia
HER2 does not help with low-grade DCIS, or with ADH vs borderline DCIS

Plasma cells are amazing! Their appearance reflects an extraordinary commitment to antibody production: the eccentric nucleus, the abundant cytoplasm filled with rough endoplasmic reticulum, the pale Golgi hof. Features we learn early in training become more interesting as we come to understand what they represent.
Some plasma cells persist for decades, producing antibodies against infections we have long forgotten. I find that remarkable. Part of our immunologic history is maintained by these cells, quietly at work in the marrow.
In neoplasia, they may retain that familiar appearance while producing a protein capable of causing profound injury. A seemingly mature plasma cell can belong to a clone whose antibodies damage the kidneys, deposit in tissues, or increase blood viscosity. Its appearance alone tells us surprisingly little about the harm it can cause.

Excited to share our team's recent study on a long-puzzling issue in lung carcinoids/NETs.
So many “aha” moments when working on it! Happy to share the lessons we learned both for the diagnostic approach & very interesting underlying biology.
