Every era has its own pressures for doctors. The long hours and in the past were tough. Today’s challenges are around capacity, admin load and retention. The constants remain the patients we care for and the responsibility we carry to deliver good health outcomes for them.

When asked how things were previously in medical practice by medical students and resident doctors, my response is “different”. Some things were better than now, some worse. Doctors of any era will face challenges; some unique and some similar to those faced by past generations.

We work with pharmacies to source alternatives or adjust prescriptions when medicines are out of stock, but these global supply issues create real anxiety and extra work for everyone. Ordering repeats at least a week in advance gives us the best chance of minimising disruption.

An experience that has become too familiar for patients. You arrive at your pharmacy to collect your medication, only to be told that the drug you need is out of stock. Read more about why this is happening and what you can do in my Daily Express article. express.co.uk/life-style/hea…

General practices should keep a protected share of appointments available for patients with lower digital confidence or complex needs. Support for carers & family to help with online access where appropriate is also important.
ClaphamHealth retweeted
Older adults and people with complex health needs are often the highest users of healthcare, yet they are the least likely to possess smartphones or have high digital confidence. When appointments are claimed rapidly online, less digitally fluent patients are at a disadvantage.

We see flu’s impact every winter: extra appointments, staff absence and pressure on services. Offering free vaccines to non-NHS eligible staff can be worthwhile: it cuts illness and absence, protects colleagues and vulnerable people.
ClaphamHealth retweeted
Should employers offer free flu vaccination for staff who are not eligible via the NHS? The arguments for are that it reduces sickness rates and therefore improve productivity. An argument against – particularly for public sector employers – is that it may not be cost-effective.

This is helpful advice. When records arrive late, incomplete or with degraded data, it slows down safe care and creates extra work for the new practice while we chase information or re-enter key details. Bringing a simple personal summary helps in these circumstances.

When patients register with a new general practice, medical records can “go missing”. It’s a good idea to keep a record of information such as diagnoses, allergies, repeat prescriptions, vaccinations, key test results, hospital letters and measurements like blood pressure.

Bacteria like E.coli are a significant cause of gastrointestinal illness.
Diarrhoea and vomiting can typically be managed at home. Make sure you rest, and drink enough fluids to prevent dehydration.

ALT E.coli bacteria under a microscope. Image has been tinted yellow. Black text on the image says, "E. Coli (Escherichia coli) bacteria are commonly found in the intestines of humans and animals. While many strains exist harmlessly in the digestive system, others may cause infections, including gastrointestinal illness. The bacterium is present in poo and can survive in the environment."

Many active patients are removed via the accelerated list-validation/FP69 process, then contact practices to say it’s an error. Re-registration is creating significant extra admin work, patients are anxious about lost access, and the capitation loss is hitting practice finances.
