When asked how things were previously in medical practice by medical students and resident doctors, my response is “different”. Some things were better than now, some worse. Doctors of any era will face challenges; some unique and some similar to those faced by past generations.

An experience that has become too familiar for patients. You arrive at your pharmacy to collect your medication, only to be told that the drug you need is out of stock. Read more about why this is happening and what you can do in my Daily Express article. express.co.uk/life-style/hea…

Older adults and people with complex health needs are often the highest users of healthcare, yet they are the least likely to possess smartphones or have high digital confidence. When appointments are claimed rapidly online, less digitally fluent patients are at a disadvantage.

Should employers offer free flu vaccination for staff who are not eligible via the NHS? The arguments for are that it reduces sickness rates and therefore improve productivity. An argument against – particularly for public sector employers – is that it may not be cost-effective.

When patients register with a new general practice, medical records can “go missing”. It’s a good idea to keep a record of information such as diagnoses, allergies, repeat prescriptions, vaccinations, key test results, hospital letters and measurements like blood pressure.

Bacteria like E.coli are a significant cause of gastrointestinal illness. Diarrhoea and vomiting can typically be managed at home. Make sure you rest, and drink enough fluids to prevent dehydration. Find out more on our blog post: ukhsa.blog.gov.uk/2026/02/06…

My practice is currently sending out a large number of emails and text messages to patients who have been removed from our list by NHS England. In many cases, they are replying to say they have been removed in error. Reregistration creates unnecessary work for our admin team.

When doctors see a patient who is acutely unwell – particularly if they are elderly, frail, clinically vulnerable or taking several drugs – they should always consider an adverse drug reaction as a possible cause of their illness when compiling a differential diagnosis.

Doctors are increasingly asked to review AI outputs they never requested. Tools that save time and help patients are welcome. Unsolicited suggestions, draft plans and safety-net prompts are not when doctors must still read, correct, explain and take responsibility for them.

Doctors do not implement guidelines uncritically. They decide which recommendations matter most, for whom and in what order. In an era when multimorbidity, polypharmacy, patient complexity and fragmented care are common, these judgments are essential. link.springer.com/article/10…
