CREST-2: In high-grade asymptomatic carotid stenosis, addition of stenting to medical therapy led to a lower risk of stroke over a 4-year period. Endarterectomy did not lead to a significant benefit. Full results: nej.md/3LK9kst

This graph sums it up. In acute stroke with tandem lesions (ICA + intracranial occlusion), acute carotid stenting during EVT was associated with better 90-day outcomes than EVT without stenting.
Outcomes were similar with or without prior IV thrombolysis (IVT).
Caveat: secondary (post-hoc) IPD meta-analysis of 6 RCTs - stenting wasn’t randomized, so results are associative, not causal.
