Kinetics of sevoflurane vaporized directly in #ECMO membrane lung
🔬ex-vivo trials using circuits primed with Ringer’s lactate; sevoflurane vaporized at concentrations of 1–8% with SGF of 1–10 L/min.
💨 sevoflurane effectively crossed PMP membranes, rapidly reaching reproducible peak concentrations to values consistent with clinical targets for deep sedation; clearance increased significantly with higher SGF
⚠️ environmental concentrations well below occupational safety thresholds
Delivering sevoflurane vaporized directly into ECMO circuits feasible/safe alternative sedative strategy. These results provide a robust foundation for translating this into pilot clinical trial aimed at optimizing ECMO sedation with inhaled sevoflurane in patients with severe ARDS.

#ECMO following acute type A aortic dissection repair?
🔍 SR/MA
🪦 successful weaning 48.3%, but high in-hospital mortality = 72.4%; late mortality 5.8% per 100 pt/y
🫘 AKI requiring CRRT 60.5%
🧠 neuro complications due to stroke 24.4%
🦵🏻 limb ischemia 7.6%
🩸 bleeding requiring reoperation 29.2%
#ECLS may be reasonable. Despite high morbidity and mortality (comparable with other PC-ECLS), myocardial recovery may be achieved with careful patient selection: young patients better candidates; pre-ECMO lactate peak appears strong predictor for mortality.

Coronary, renal & cerebral perfusion of VA #ECMO + #IABP in refractory cardiogenic shock
🔬in vitro study
🫀hemodynamic effects depend on CO, HR, site of #ECLS arterial cannulation
If severe low CO, axillary artery (vs femoral) cannulation + IABP showed greatest benefit in terms of coronary/cerebral flow but renal perfusion negatively affected.

One important point about differential hypoxemia:
People often think the R radial sat will be the first site to show drop. This isn’t true.
The first vessel to experience hypoxemia It’s the coronary arteries.
Watch for cardiac ischemia as a sign of differential hypoxemia!

Normothermic machine perfusion is increasingly used to approximate human physiology, preserve organs outside the body, provide dynamic graft assessment. Although NMP application is relatively new, its concept, foundation, development can be traced back to 1930s:
