Hostile iliacs during open TAAA?
➡️Consider a bottom-up approach!
✅Upfront reconstruction of iliac anatomy
✅Shorter duration of renovisceral LHB
✅Forgo femoral chimney
Happy to contribute to this great work, led by David Stonko & @JamesHBlackMD !

We stand on the shoulders of giants, and Dr. Perler was one of those giants. His impact as a surgeon, educator, editor, and leader shaped generations of vascular surgeons. His legacy will endure through the many trainees, colleagues, and patients he influenced. Rest in peace.
The Society for Vascular Surgery mourns the passing of Bruce A. Perler, MD, MBA, FACS, DFSVS, a past president of SVS and a leader whose contributions helped shape the future of vascular surgery.
During his 44-year tenure as a surgeon at The Johns Hopkins Hospital, Dr. Perler established the Division of Vascular Surgery and Endovascular Therapy. He trained generations of surgeons in the art and science of vascular surgery. His humble demeanor, prodigious skill and evident mastery are cherished by all the Halsted residents who knew him and live on as his legacy in American surgery.
Dr. Perler was internationally recognized for his clinical expertise in carotid arterial disease, having delivered nearly 400 lectures nationally and internationally. His hundreds of publications, research and chapters have been cited over 12,000 times.
In addition, he served as Senior Editor of the Journal of Vascular Surgery, co-edited three editions of Rutherford’s Vascular Surgery and Endovascular Therapy and held leadership roles with the American Board of Surgery and several vascular societies.
Dr. Perler received the 2025 SVS Lifetime Achievement Award, the Society’s highest honor, recognizing a career defined by exceptional clinical care, academic leadership and enduring contributions to the advancement of vascular surgery. This was his third lifetime achievement awarded by his colleagues in surgical and interventional societies, a fitting tribute for a career that reflected not only clinical excellence and international leadership, but a deep understanding of the evolution of the field of vascular surgery over the past decades.
Dr. Perler is survived by his wife Patti and his daughter Rachel. His son Mason passed away in 2024. SVS extends its deepest condolences to his family, friends and colleagues.

Still can’t believe it’s come to an end! Grateful for all of these amazing people in the past two years and excited to start my next journey @MayoClinic Jacksonville!
Yesterday we celebrated our graduates Dr. Kim Gerling and Dr. Wen Kawaji. We are incredibly proud and look forward to the amazing journeys ahead of them. We will miss you! Don’t forget your Hopkins vascular family is just a phone call away. #ThisIsHowWeVascular #VascularFam
100% agree. I learned by 1) interpreting the CT, 2) reading the radiology report, and 3) going over the CT with the radiologist in the reading room if any discrepancy / confusion.

Proud to play a part in his training. He’s going to be an excellent vascular surgeon!

And just like that, Dr Alex Krosser has completed the formal part of his vascular surgery training. Congrats! @JeffersonUniv @JEFFsurgery @JEFFAortic @JeffVascSurg @JeffersonVSIG @pdimuziomd Dawn Salvatore @mnooromid @AruRobbie
In our recent paper in @JVascSurg , we found that the risk of CCA dissection (1.2%) during #TCAR doubles when the ECA is not selected. CCA dissection most likely occurs during sheath insertion.
Bottom Line: Always select the ECA when feasible!

It's that time of year to pass along whatever tips you can....You control only one thing, your integrity. Do not compromise it.
Grateful for the opportunity at VAM to share our volumetric research in post-dissection TAAA and its association with TVI after BEVAR (t-Branch).
📖TVI was associated with:
✅Volume of the paravisceral TL
❌Diameter of the paravisceral TL, FL origin, or TV dissection

Proud to mentor 3rd year medical student Gerard Martin to his first VAM poster presentation on “Temporal Trends in Demographics Among General and Vascular Surgery Applicants and Trainees.”
He did a fantastic job from start to finish - bravo Gerard!
