Grateful to @EricTopol for picking this up 🙏. The other half of the argument came out the same week in Advanced Materials: what conjugation costs the antibody itself, and why a program should state whether it is building a drug or a delivery vehicle.
This field is hot: the pace of innovation of antibody-conjugate drugs vs cancer is so accelerated that clinical trials can't keep up nature.com/articles/s41591-0… nature.com/articles/d41586-0…

Two Perspectives on antibody-drug conjugates, published days apart, one from the clinic and one from the chemistry. I am struck by how differently the same problem looks from each end.

Since the dose ceiling is set by payload toxicity, not the antibody, that loss cannot be made up by giving more. Whether it matters depends on what the antibody was there to do: a great deal for trastuzumab in HER2+ disease, far less for a pure delivery backbone.

What neither can yet say is how much the antibody still contributes once conjugated, for which targets and in which settings. No trial design in use separates that from the payload. @GustaveRoussy #institutstrauss @ERC_Research @laliguecancer @Inserm @ParisSaclay
