The "chicken-or-egg" debate in Idiopathic Intracranial Hypertension (IIH): Does elevated ICP cause venous sinus stenosis, or vice versa? 🤔
The rapid resolution of VSS post-LP strongly supports the Starling Resistor model—extrinsic compression creating a functional stenosis. Venous stenting is highly effective because it permanently breaks this vicious cycle! 🧠⚡️

New consensus guidelines on imaging for spontaneous intracranial hypotension (SIH):
-Start with a brain MRI (with contrast)
-Add a spine MRI to look for possible CSF leaks
-Use advanced imaging to pinpoint the leak when needed
-Keep in mind: imaging can be normal even when SIH is present
-Clinical judgment remains key in diagnosis and care.

Mastering thalamic vascular variants is crucial in stroke imaging! 🧠 Let's review the 4 patterns of paramedian thalamic supply:
🔹 Type I: Symmetrical independent arteries (most common normal anatomy)
🔹 Type IIa: Asymmetrical origin from a single PCA
🔹 Type IIb: Artery of Percheron (A single common trunk bifurcating to supply bilateral thalami) ⚠️
🔹 Type III: An arterial arcade pattern connecting the PCAs
Recognizing the AOP (Type IIb) is critical in the reading room—occlusion means bilateral thalamic infarcts from a single vessel!

Just because it’s called SMALL vessel disease doesn’t mean it doesn’t have a BIG impact!
Small vessel disease (SVD) is a BIG contributor to vascular dementia, along w/large vessel cortical infarcts
Do YOU know all the faces of small vessel disease?
Common imaging markers of SVD include:
--White matter hyperintensities (WMHs)
--Lacunes
--Enlarged perivascular spaces
--Microbleeds
--Small subcortical infarcts
--Brain atrophy
But what you see isn’t always what you get!
Even in patients w/similar degrees of SVD, clinical symptoms can be very variable.
Variability exists because of:
--Perilesional penumbra
--Remote effects from disruption of brain connectivity
--Differing brain reserve & compensatory mechanisms
So remember, imaging findings of SMALL vessel disease are just the tip of a very BIG iceberg
