In this RCT, in nonurban areas in Catalonia, Spain, there was no significant difference in 90-day neurological outcomes btwn transportation to a local stroke center vs a thrombectomy-capable referral center in pts w suspected large-vessel occlusion stroke. ja.ma/3LJRkcJ

Latest in basic science - bit.ly/3MqIcd7 - Li J et al showed up to 79% variability in devices’ tracking performance. Hybrid coil and braid reinforcement configuration and DACs’ distal wall thickness are significant contributory factors to ease navigation. @marcriboj

Dr. Jovin is right, we determined the detrimental effect of additional passes, recanalization with any passes is still better than NO recanalization. When to Stop Detrimental Effect of Device Passes. doi.org/10.1161/STROKEAHA.11…
This proposal reflects flawed logic. Ample evidence exists that # of passes correlate with outcomes BUT also that reperfusion is better than no reperfusion. Outcomes in > 3 pass pts should be compared to non treated patients and not with ≤ 3 pass pts. medpagetoday.com/neurology/s…

Endovascular Therapy for Stroke Due to Basilar-Artery Occlusion | NEJM nejm.org/doi/full/10.1056/NE… Inability to show a significant difference in outcomes does not mean there is no difference. Had the study not been redesigned mid trial, results would likely have favored thrombectomy.

Direct to Angio Suite (DTAS) workflow on stroke patient: Admission NIHSS= 16 3 hours since stroke onset LVO suspected Door to reperfusion (Aspiration): 18 minutes NIHSS 24h= 1 Every 30 minutes delay before treatment reduces the chance of a good outcome by 14%
Should we trust CTP in the hyperacute period? @TudorGJovin @SunilAShethMD ? We all know the answer ..
