Yes, this is a relatively small patient cohort, but our systems are not well designed to care for them. The end result is long stays in an environment that makes people worse. We can do better

Increasing numbers of mental health patients are being seen in A&E – with a growing number waiting 12 hours or more to be seen in what is often not the best environment for their recovery
@RCEMpresident says patients ‘turn up because alternative access to help is not good’ [5/11]
Pet hate: emails that start with ‘This is a gentle reminder…’ Perfectly happy with just the reminder! 😉😂

Dear Jr Drs that I manage.
If u booked leave, & we agreed it - go on holiday and forget about work (& don’t use ur phone driving)
If another dr didn’t turn up for the shift -it’s my problem and not yours!
It’s worrying @NHS_HealthEdEng. think this is correct guidance

Latest children's trauma report from @TARNaudit. Self harm seems to have recurrent September and May/June peaks. Tempting to relate to new school year and summer exams.
NOTE: This is only trauma - almost all asphyxia (hanging) - and excludes pre-hospital deaths.
This makes a lot of sense👇🏻

1/n Let's grab a cuppa and chew over this. First off, patients in emergency departments are experiencing very long stays, at a scale that is much worse than we've ever seen. There were more 12 hour (from decision to admit) waits in Sept 22 than ever before.

This just never makes sense to me - as all your are saying is let’s keep the status quo of accumulating risk mainly in ED – at Manchester, England

The “cool, fun, call me Tony, I’m-not-like-a-regular-consultant” consultant when his financial adviser asks him why he spends £2000 a month at the hospital Costa
Fantastic talk from Prof Steve Goodacre on the tension between diagnostic and prognostic aspects of risk scores. Classically we think the below 👇🏻

Get ready for some tweets from Belfast's #RCEMASC - first up is the Toxicology stream - nitrous misuse is increasing, as is cocaine, mephedrone use is down

As a society we have choices.
Our systems are causing excess deaths

Take home from the excellent Paul Atkinson:
Fixing EM requires resources, skills and training and compassion…
In this broken system we must clarify our primary position: what is on your business card as an emergency physician?
Outgoing RCEM president messages:
1. We have to be make time and be able to train staff.
2. Maintain your self worth - do the right thing for you, patients and colleagues.
3. Balance bravery and risk - there are times for both
4. Look after the team

I love this thread on the current drugs vs talk therapy debate, particularly the bit that goes ‘Do you offer a drowning person a life belt or an ideology-fuelled monologue about how the drugs don’t work?’
It is amazing how difficult this is to achieve despite the unimpeachable logic…
It is beyond shocking that we are now living in a country where taxation rates and culture wars trump (deliberate pun) the combined existential threats of Russia and the climate emergency… apple.news/AhkSwSO2IRmaR1jVt…

We took the plunge and scrapped total triage 2 weeks ago. The relief is immense for GPs, receptionists and, from what I’ve heard so far, patients. Can choose F2F or phone. Access via E-consult, phone or walk in. Also gone to 15 min appointments.
Back to the job I love. #TeamGP
