Thanks for all the good thoughts, this paraganglia! It’s important to recognize that they can be incidentally found in GB sections to avoid misdiagnosis with neoplasm. This paper describes it beautifully with the IHC work up if needed.

35-year-old patient with multiple firm, skin-colored to erythematous papules and nodules on the face, scalp, and neck. Lesions have progressively increased in number. Biopsy shows well-circumscribed dermal epithelial nests with peripheral basaloid cells and a characteristic “jigsaw puzzle” pattern.
🩺 What is the diagnosis?

“Madina Azahara”, Cordoba, Spain - A deeply moving experience visiting the ruins of this magnificent place earlier this morning. A city born from ambition, destroyed by history and immortalized by beauty. The city where Abu al-Qasim al-Zahrawi (Abulcasis), the great Andalusian surgeon practiced as the royal physician (credited with performing the first thyroid FNA, often referred to as the Father of Surgery).

BAGP Case of the Month 🔬
Are you ready for this week’s pathology puzzle 😉? This one is a ‘small’ tumour...but a big diagnosis.
33-year-old female presenting with pelvic pain and an 11cm ovarian mass.

🏰 The PASH story
The Pasha lives in a very DENSE palace (dense collagenous breast stroma)
Inside the walls, his MYO workers (myofibroblasts) are digging lots of winding, interconnected empty tunnels

5 Things Every Pathology Resident Should Know About Invasive Breast Carcinoma NST
3. E-cadherin isn’t enough
E-cadherin is one of the most useful immunostains when distinguishing invasive breast carcinoma NST from invasive lobular carcinoma—but it should never be interpreted in isolation.
A panel is often more informative:
- NST: membranous E-cadherin, β-catenin, and p120 (Case A 👇)
- Lobular carcinoma: loss of E-cadherin and β-catenin, cytoplasmic p120 (Case B 👇)
Be aware of aberrant staining patterns. As always, morphology comes first—immunohistochemistry should support, not replace, the diagnosis.

INTRANODAL PALISADED MYOFIBROBLASTOMA.
This rare benign lymph node-based myofibroblastic tumor is associated with recurrent CTNNB1 mutations. Beta-catenin IHC (shown) can be helpful to confirm the diagnosis.
