continuing empiric vancomycin for >48 hours without *any* evidence of MRSA (no PCR, culture, or gram stain data to support the presence of MRSA) should be a federal crime.

On this day 16 years ago, the NICE SUGAR trial was published, showing that intensive glucose control increased mortality among adults in the ICU. Today, we continue to target a glucose of less than 180 mg per deciliter owing to the results of this study. TIL @ATCMeeting: 65.9% of transplant pharmacists meet criteria for #burnout - more than any other discipline in the field. Sounds like a #CallToAction is imperative at this point!

new chapter: ground glass opacities (GGO) ☁️ GGO is enormously common (and no, it doesn’t mean that the patient has pneumocystis or COVID 🙄) ☁️subtypes help narrow the differential (eg diffuse GGO, mosaic pattern, crazy-paving, headcheese sign) emcrit.org/ibcc/ggo/ #IBHP

fresh chapter: hilar & mediastinal lymphadenopathy 🔍 mild swelling of a couple nodes is nonspecific, but bulky lymphadenopathy is diagnostically important 🔍distribution, imaging characteristics, & context can help sort the possibilities emcrit.org/ibcc/lymphadenopa…

fresh chapter: septal thickening, crazy paving, and fibrosis 〰️ septal thickening often suggests heart failure, but the differential is broad 🤪 approach to the acutely ill patient with crazy paving 🐝🍯 causes of honeycombing emcrit.org/ibcc/reticular/#t…

fresh chapter: cavitary lung lesions! 🕳️ broad differential (the usual suspects: infection, malignancy, inflammatory) 🕳️ key clues include number, location, wall thickness, surrounding lesions 🕳️ air-crescent sign suggests aspergillus, but not always emcrit.org/ibcc/cavitation/

fresh chapter: distribution of radiologic abnormalities 👀 migratory abnormalities that shift rapidly argue against infection 👀 peribronchovascular dist is subjective, but if unequivocal can help 👀 central & peripheral distributions are seen often emcrit.org/ibcc/distribution…

fresh chapter: nodular patterns 😃 ☘️ different patterns can have immediate & major implications ☘️ acutely ill pt w/ miliary pattern suggests TB or fungus. vanc-piptazo won’t fix this. ☘️ MIP (maximal intensity projection) images often help sort out emcrit.org/ibcc/nodule/#top
